Abstract (EN)

Introduction and Aim: Coronary artery disease (CAD) and acute coronary syndromes are among the most common causes of death in our country and the world today. Although these diseases, which arise on the basis of atherosclerosis, have multifactorial aspects such as genetic and environmental, recent studies have shown us that atherosclerosis is an inflammatory process. Because of the high mortality and morbidity rates of acute coronary syndromes, it is important to determine and manage the prognosis well. For this reason, many scoring systems have been developed to determine the extent and prognosis of the disease, and even to guide their treatment. The most commonly used scorings are syntax and gensini scoring. In our study, we aimed to determine whether there is a relationship between inflammatory markers and the prevalence of coronary artery disease determined by scoring in patients with acute coronary syndrome . Material and Methods: A total of 190 patients with the diagnosis of acute coronary syndrome admitted to the Cardiology Coronary Intensive Care Service of Recep Tayyip Erdoğan University Faculty of Medicine for 6 months between 15.04.2021 and 01.10.2021 were included in this study. The name, gender and age information of the patients participating in the study were obtained. The patients' backgrounds were investigated for additional hypertension, diabetes mellitus, hypercholesterolemia, and coronary artery disease. In addition to these, the presence of family history in terms of cardiovascular disease, smoking and obesity were questioned. Coronary artery disease prevalence was determined by scoring systems by recording the information after coronary angiography performed during the treatment of the patients. They were grouped according to their scores. It was investigated whether there was a relationship between inflammatory marker values measured during hospitalization and scoring classifications. Results: Of the 190 patients included in the study, 16.3% (n=31) were female and 83.7% (n=159) were male. Their ages ranged from 32 to 89 years, with a mean age of 61.09±11.39 years. In our study, it was observed that the basic viii inflammatory markers were higher in patients with high Syntax and Gensini scores. A statistically significant positive correlation was found between SYNTAX and CRP, WBC, Neutrophil count, Neutrophil/Lymphocyte ratio (NLR) and CAR, respectively (with SYNTAX, CRP; p=0.013; WBC p=0.015; neutrophil count p=0.004; NLR, p=0.002; CAR; p=0.003). A relationship with Gensini scoring in a similar direction was found with CRP; p=0.006, WBC; p=0.007, neutrophil count; p=0.025, CAR p=0.001 being significant. Conclusion: A statistically significant and positive correlation was found between the calculated SYNTAX and Gensini scores and inflammation markers, which CRP, White Blood Cell (WBC), Neutrophil count, Neutrophil/Lymphocyte ratio (NLR) and CAR, respectively in the patients in our study.
