Obstetric outcomes of mother-friendly and non-mother-friendly hospital practices

In our age, when the idea that birth is physiological has begun to decline, the Mother-Friendly Hospital Program has been established to raise the standard of pregnancy services based on evidence, prevent perineal complications, and reduce the cost. The purpose of our studyis to determine the obstetric outcomes of mother-friendly and non-mother-friendly hospital practices. In our descriptive study, 176 women who gave birth between January and April 2019 in the birth room of Trabzon HSU Kanuni TRH Numune Campus were included in the NMFHP group, and 180 women who gave birth between October and December 2019 were included in the MFHP group. The data were collected with a face-to-face interview and continuous observations using the "Data Collection Form". In the statistical analysis of the data, percentage, mean, standard deviation, median, minimum, and maximum values, Chi-Square, t-test, Mann-Whitney U test, Pearson Chi-Square, and Fisher's Exact analyzes were used. The mean age of the women in the study was 28.6±5.25 years. In the MFHP group, the frequency of vaginal examination (4.98±1.79 times), intervention to accelerate birth without 4 cm dilatation (11.7%), using enema (1.1%), restriction of oral liquid food (14.4%), fundal pressure (11.1%), episiotomy (46.1%), additional uterotonic for postpartum (48.3%), and neonatal soft tissue trauma (36.7%) were found to be lower than the NMFHP group (p<0.05). Besides, in the MFHP group, the ratio of introducing the service (76.7%), providing information about the 1st stage (93.3%), the 2nd stage (99.4%), the duration of the 2nd stage (97.8%), the 3rd stage (97.2%) and the postpartum period (92.2%), using non-pharmacological methods (98.9%), providing early skin-to-skin contact (98.3%), providing hands-on breastfeeding support by the midwife/nurse (81.1%), satisfaction with the hospital (98.3%), early initiation of breastfeeding (0.39±3.28 min), abdominal uterine tone evaluation in the early postpartum period (2.46±0.88 times), vital signs follow-up (2.21±1.14 times), vaginal bleeding-fundal height control (2.47±0.87 times) were found to be higher than those in the NMFHP group (p<0.05). In conclusion, it was determined that the Mother-Friendly Hospital Program was effective in increasing the practices supported by scientific evidence, reducing the unsupported practices, and improving mother-infant outcomes.
