Abstract (EN)

Introduction and Aim: The aims of this study were to investigate bacteremia frequency of children who came to Turgut Özal Medical Center Pediatric Emergency Department with complaint of fever between June 2012 and October 2013, to determine the patients having the risk of bacteremia while waiting for the results of blood culture by evaluating the serum parameters of bacteremia (the number of leucocyte, CRP, SAA, TNFα, IL-6), and to decline the risk of complication with suitable clinic application and modality. Appliance and Method: The sample group of the study was 380 patients who came to İnönü University, Turgut Özal Medical Center Hospital Pediatric Emergency Department with complaint of fever and whose blood cultures were taken. From the file records of these patients the results of their blood cultures, throat and urine cultures (if sent), and acute phase reactants were taken, (WBC, CRP, IL-6, TNFα, and SAA) and the relation between acute phase reactants and culture results were studied. Findings: 158 (%42) of the patients were female and 222 (%58) of them were male. The rate of male/female was 0, 7. Their ages were between one month and 203 months. Average age was found as 56 months and median age as 46 months. In 22 of sent blood cultures reproduction was stated. Patients' frequency of bacteremia was identified as %5,7. When the patients who have and don't have reproduction in blood culture are compared in terms of acute phase reactants; statistically no meaningful correlation between two groups was found. When urine culture results of some patients evaluated, reproductive and non-patients when compared in terms of acute phase reactants There was no statistically significant relationship between the two groups. Again when throat culture results of some patients evaluated, no significant correlation was found statistically between the patients who have and don't reproduction from the point of WBC, IL-6, TNFα, SAA. A significant difference (p= 0,003), in CRP results of the patients who have and don't have throat culture reproduction, was stated. When culture results are evaluated in a cumulative way, statistically no significant difference was found between the patients who have and don't have reproduction in any culture with regard to WBC, IL-6, TNFα, and SAA. Statistically significant difference was found in the CRP results of the same group. (p=0,003). Result: CRP rate of the patients who have reproduction in throat culture is found statistically significant when compared with the patients who don't have reproduction in throat culture, that's why the positive CRP results of patients having infection findings in oropharynx and/or tonsils should be thought as the probability of bacterial infection.
