Evaluation of hemodynamic changes which are associated with karotid artery, vertebral artery and retrobulbar circulation with color doppler ultrasonography in migraineurs with and without aura

Özet (EN)

In our study, we aim that evaluation of hemodynamic changes in karotid artery, vertebral artery and ocular arteries in migraineurs with and without aura with utilising color duplex doppler ultrasonography.64 patient whose diagnosis are migraine and 36 healthy control group are included to our study in policlinic of norology, Bülent Ecevit University. Migraineurs are classified with or without aura according to International Classification of Headache Disorders. Migraineurs are evaluated in term without headache. Measurements are did for each karotid, vertebral artery and eye in every participant. We search intima media thickness in karotid arteries, volum flow and diameter in vertebral arteries. We measure peak systolic velocity(PSV), enddiastolic velocity(EDV), middiastolic velocity(MDV), maxımum velocity(TAMAX), peak systolic velocity(PSV)/ enddiastolic velocity(EDV), enddiastolic velocity(EDV)/ peak systolic velocity(PSV) in ophtalmic artery (OA), central retinal artery(CRA) and posterior cilier artery which are in retrobulbar circulation.In our study, among all migraineur group, TAMAX in right CRA, PSV/EDV and EDV/PSV in right OA and PSV in left OA are statistically found significant high in control group. PI value in right CRA are statistically found significant high in migraineurs without aura than migraineurs with aura. EDV / PSV value in left CRA, EDV and MDV value in left OA statistically found significant high in male migraineurs than female migraineurs.Vertebral artery diameters are statistically found significant low in female migraineurs than male migraineurs and control group. İn control group and migraineurs, karotid intima media thickness and volum flow of vertebral artery are not statically significant.Eye pressure of migraineurs are found in normal limit.The aim of our study is to evaluate vascular hemodynamic changes of karotid, vertebral and ocular artery with color duplex doppler ultrasonography in migraineurs and to compare doppler findings among each different migraine group and control group. As a result, in our study, we did not meet with meaningful finding.Also a few resembling study in literature, cerebrovascular hemodynamic changes in migraineurs were found. According to results, at the time of migraine headache, retrobulbar hemodynamia make a response than intracranial arteries.In literature, study that research ocular hemodynamic changes are comparatively small amount.In our study, we determined meaningful changes in ocular and vertebral system rather than karotid system. We think about changes which are in ocular system are associated with vascular structure and otonomic dysfunction. Hypoplasia in vertebral artery can cause hipoperfusion of posterior circulation so neurological symptoms arise. Because of the insufficient studies, more studies are needed to perform with wide patient series.
