Abstract (EN)
The aim of this study is to evaluate the effect of position change on pain in unconscious patients with stroke. The descriptive cross sectional study was held between 16 August 2021 and 16 February 2022 on a city training and research hospital in the South Marmara Region. It was consisted of patients with stroke, treated in the intensive care unit in the research, data were collected from Data Collection Form, Intensive Care Pain Observation Scale, Behaviral Pain Scale, Patient Follow Up From: Giving Position and Motor Activity Rating Scale. The necessary in stitutional permission and ethics committee permission were obtained for the study, and an informed can sent form was signed by the individuals participating in the study. The power of the research was determined according to the Post Hoc Power analysis. p<0.05 was accepted as statistical significalce level. Unconscious patients with stroke in the intensive care unit were 56.62% women, 40.3% 70-79 years old group, 68.2% of them married. 26.4% of the patients were high school graduates. 42.6% were urgently needed in the intensive care unit for 11-20 days. 54.3% took analgesics medication and 33.3% of them used analgesics regularly. Patients in the study 63.6% were intubated 36.4% had pressure ulcers, 17.8% had central venous catheters, 76.7% had urinary catheters.Almost all patients were diagnosed chronıal disease, in 53.5% hypertension, 43.4% previous operation history, 43.4% physical restriction have been determined. Of the patients, mean total score on the Intensive Care Pain Observation Scale was 2.68±2.74, Behavoral Pain Scale mean total score was 4.41±1.94, Motor Activity Rating Scale mean total scorre was 2.47±2.06 found, regularly. In the study, before-during and after position change, the heart rates, blood pressures, oxygen saturation values, changes in pupils, sweating, nausea-vomiting, skin paleness and redness were evaluated and observed that there was a significant difference (p<0.05). In the multiple comperisous, it was determined that the values before and after the procedure were lower than the values during the position change. Physiological changes and findings during position change in stroke patients in the intensive care unit who are unconscious (blood pressure, heart rate, oxygen saturation, sweating, skin redness, nausea-vomiting) and physical recations (such as facial and extremity movement adaptation to ventilation, restlessness) supported that they felt pain. It is ve commended to diagnose pain in the intensive care unit at frequeut intervals and with reliable scales, to apply pharmacological and nonpharmacological methods before position change and to provide in-service training to working nurses to manage pain behavi our.
