Effects of the active vitamin d treatment on proteinuria and gfr in renal transplant recipients

Abstract (EN)

BACKGROUND: In renal transplant patients it was found that vitamin D deficiency is common. Outside the bone and mineral metabolism of vitamin D, vitamin D found in many tissues Receptor (VDR) through non-genomic effects are realized. Vitamin D regulates lipid metabolism in the liver, to balance the levels of insulin, by the ability to inhibit renin angiotensin aldosterone system (RAAS) protected from cardiovascular disease and hypertension. AİM: İn this study, we aimed to compare renal transplant patients with active vitamin D supplementation and control group, creatinine clearance, proteinuria. If there's positive findings as a resarch result , vitamin D replacement therapy can be add to treatment program without aiming any parathormon level for increasing the lifetime of kidney. MATERİALS AND METHODS: The study was performed prospectively with 124 kidney transplant patients. Active vitamin D treatment was given 25 micrograms a day to the nondiabetic petient that parathormon level over 400.And other petient that parathormon level under 400 were obserrved . Vitamin D level,complet blood counting and immunsupressive drug level eveluated in all patients.Vitamin D level eveluated with HPLC (high-performance liquid chromatography) method. FINDINGS: In patients that given active vitaminD treatment, proteinuria level was 158,09±58.84 miligram/day at 0th month, 123,32±51.74 miligram/day at 6th month(p: 0.02).In patients that given active vitamin D , glomeruler fitration rate was 67.5926.13 milliliter/minute/1.73 square meters at 0th month, 76.45±14.23 milliliter/minute/1.73 square meters at 6th month (P: 0,000). Vitamin D level was 8.6±6.2 nanograms/milliliter at 0th month, 19.14±7.02 nanogram/milliliter at 6th month (P:0,000) . RESULTS: There was negative correlation between vitamin D level and proteinuria in patients that receiving active vitamin D therapy(P<0,001).And we detected positive correlation between vitamin D level and glomeruler filtration rate(P<0,001).
