Association of hospital length, age and gender with biochemical, inflammatory and hematological parameters in service patients entered with COVID-19 diagnosis

Abstract (EN)

The COVID-19 pandemic has adversely affected hospitals and health systems all over the world, both in terms of service and finance. Factors such as length of hospital stay, age and gender are very important in predicting health care resource use and cost in COVID-19. Therefore, in our study, we retrospectively evaluated the laboratory parameters at admission to the hospital, as well as the length of hospital stay, age and gender factors of 1036 COVID-19 patients in the COVID-19 service of Ankara Pursaklar State Hospital. Higher AST/ALT, hyponatremia were seen in patients who stayed in the hospital for more than 7 days. At the same time, strong positive correlations were observed between ferritin, fibrinogen and LYM values and hospital stay. Significant differences were observed in almost all biochemical, inflammation factors and hematological data depending on age. Significant negative correlations between age and AST/ALT, EGFR, sodium, RBC, HBG, EOS, whereas strong positive correlations were seen between age and uric acid, BUN, LDH, potassium, CRP, CLR, D-Dimer, Fibrinogen, HCT, WBC, NLR, NEU, MON and PDW. It was observed that AST, ALT, T. Bilirubin, EGFR, RBC, HBG and almost all inflammation and coagulation factors were higher in men compared to women, whereas PCT, PLT and RDW values were significantly lower. In the ROC (Receiver operating characteristic) analysis, it was seen that BUN (AUC=0,810) and CRP (AUC=0,767) for patients over 65 years of age, ferritin (AUC=0,747) and HBG (AUC=0,803) parameters for male gender are parameters that can be used to predict the disease. As a result, it was seen that AST/ALT is an important parameter that affects both the length of hospital stay and age. In addition, it was observed that male gender had more negative results than females in almost all laboratory parameters, especially inflammatory and coagulation parameters.
