Abstract (EN)

Purpose: Testicular torsion, rotation of the testis with bending of the longitudinal axis of the spermatic cord, is a surgical emergency that requires prompt treatment to ensure survival of the affected testis. Testicular salvage rates depend on the degree of torsion and the duration of the accompanying ischemia. Recently, the role of hematological parameters in the diagnosis and differential diagnosis of testicular torsion has been investigated. In this study, the relationship between hematological parameters and patient outcomes in patients diagnosed with testicular torsion will be investigated. Materials and Methods: This study was planned as a descriptive cross-sectional study. The population of the study will consist of patients who applied to Dicle University Medical Faculty Hospital Adult Emergency Service between 01.01.2010-31.12.2021. In this retrospective, descriptive study, the files of approximately 250 patients were scanned and the demographic characteristics of the patients (age, date of application), whether they were accompanied by trauma, comorbid diseases, laboratory parameters (Hb, WBC, PLT, Neutrophil, Lymphocyte, Glucose, Albumin, Globulin), affected testis, bilateral or unilateral, examination findings, time from admission to the emergency room to hospitalization, treatment method, diagnosis method, patient outcome will be examined. The conformity of continuous variables to the normal distribution was examined using the Shapiro-Wilk test. Continuous variables using mean ± standard deviation or median (minimum: maximum) values; categorical variables were expressed as n(%). ANOVA test was used in case of conformity with normal distribution and Kruskal Wallis test was used in case of non-conformity to normal distribution. In case of general significance after Kruskal Wallis test, subgroup analyzes were carried out using Dunn-Bonferroni test. Categorical variables were compared between groups using Chi-square test, Fisher's exact chi-square test and Fisher-Freeman-Halton test. SPSS (IBM Corp. Released 2012. IBM SPSS Statistics for Windows, Version 21.0. Armonk, NY: IBM Corp.) program was used for statistical analysis, and type I error level was accepted as 5% in statistical analysis. Results: When the outcome-based study group was formed, the results were determined as follows: It is seen that there is a difference between the study groups according to age (p=0.003). It was determined that the median age of the patients who underwent surgery with orchiectomy was higher than the orchiopexy group (p=0.002). It was observed that the incidence of swelling symptoms differed between study groups (p<0.001). There was a difference between the study groups in the incidence of redness symptoms (p<0.001). It was determined that there was a difference between the patients' time to the intervention and the study groups (p<0.001). It is seen that there is a difference between the study groups according to the hemoglobin value (p=0.037). It is seen that there is a difference between the study groups according to the WBC value (p<0.001). It is seen that there is a difference between the study groups according to the neutrophil(%) value (p<0.001). It is seen that there is a difference between the study groups according to the lymphocyte (%) value (p<0.001). It is seen that there is a difference between the study groups according to the CRP value (p<0.001). It is observed that there is a difference between the study groups according to the glucose value (p=0.006). It is seen that there is a difference between the study groups according to the albumin value (p<0.001). It is seen that there is a difference between the study groups according to the globulin value (p<0.001). It was observed that there was a difference between the study groups according to the neutrophil/lymphocyte value (p<0.001). It is seen that there is a difference between study groups according to albumin/globulin value (p<0.001). It is seen that there is a difference between the study groups according to the MPV value (p<0.001). Conclusion: Previous literature studies were generally conducted on experimental animals and were evaluated histo-pharmacologically. Our study is at a level that can be considered the first in this field, and it is valuable because of the limited number of patients and the lack of similar studies despite the selection of adult patients. The statistical values we try to give in the findings section are valuable and set an example for every future study.
