COMPLETE ARTERIAL REVASCULARIZATION USING RADIALARTERY IN DIABETICS WITH CORONARY ARTERY DISEASESUMMARYOBJECTIVE: The prognostic advantage of arterial grafts appear to be particularlyhigh in patients with diabetes mellitus (DM). The aim of this study was to demonstrate theinfluence of complete arterial revascularization (CAR) in diabetics with coronary arterydisease.METHODS: From September 2001 till November 2004, 238 consecutive diabeticpatients underwent isolated CABG utilizing cardiopulmonary bypass. These patients?records were retrospectively reviewed. CAR was performed using left internal thoracicartery (ITA) and one or two radial arteries (RA). The early-term outcomes of 91 patientsundergoing CAR (Group 2) were compared to those of 147 patients not undergoing CAR(Group 2).RESULTS: Demographic data for patients in both groups were similar. Mean distalanastomosis number was similar between two groups (3.14 ± 0.9 versus 3.19 ± 0.9). Onaverage, Group II was similar to Group I in terms of ITA grafting (98,9% versus 96,6%)and less likely to have RA grafting (33% versus 99%). The rate of using bilateral RA inGroup 2 was 56%. There was no significant difference in operative mortality (1,4% versus1,1%), need for IABP (4% versus 3%), stroke (1.1% versus 2,1%), reexploration forbleeding (2% versus 4%) and deep sternal infection (1% versus 1,1%). However, the needfor inotropes after 48 hours postoperatively was higher in Group 1 compared to Group 229(p<0,05). I Hospital stay was not significantly different between two groups but ICU staywas significantly higher in group 1 than group 2.CONCLUSIONS: These results suggest that CAR using left ITA and single orboth RA conduits in diabetics is safe.Key Words: Diabetes mellitus, complete arterial revascularisation, radial artery
