Comparison of Caudal Anesthesia Efficacy Applied with 20 and 30 mL Solution in Adult Orthopedic Surgery. Epidural anesthesia from Central blocks is quite widely used. Caudal anesthesia is also a method of epidural Anesthesia applied by entering the epidural distance from the sacral hiatus. Caudal applications are more widely used in pediatric patients. But most of them are also for the purpose of analgesia and in combination with general anesthesia. Its common use in adults is mainly for the management of chronic low back pain. There is not enough literature data for surgical anesthesia in adults. Naturally, literature data on the dose and volume of caudal epidural anesthesia is also insufficient. In this study, we aimed to compare the effectiveness and safety of caudal anesthesia in adult orthopedic surgery under the knee in volumes of 20 and 30 ml. For this study, records of 60 ASA classification I, II and III patients aged 18-65 who underwent elective knee surgery under caudal anesthesia were retrospectively evaluated. From consecutive patients, two groups were formed so that 30 patients would fall into each group according to the application of 30 mL and 20 ml caudal volume. Ansesthetic solutions were prepared to be 14 mL of 2% lidocaine, 3 mL of NaHCCO3, 2 mL of fentanyl, 1 mL of 0.05 mg of adrenaline in 20 mL. Demographic data of the groups, hemodynamic parameters, block levels, anesthesia quality and complications were evaluated. Demographics were similar. Block levels were T10 (T10-T8) in the 30 mL group and T12 (T12-T10) in the 20 mL group and there was a statistically significant difference (p<0.001). No statistical difference was found in the quality of anesthesia, hemodynamic parameters and complications. As a result, this study showed that a volume of 20 ml of anesthetic solution in adult orthopedic operations provides anesthesia as effective as higher volumes.
