Conservative and surgical treatment results in adult tibia diaphyseal fracture

ABSTRACTCONSERVATIVE AND SURGICAL TREATMENT RESULTS IN ADULT TIBIADIAPHYSEAL FRACTURESeventy five adult patients who have tibia diaphysis fractures applied toClinic of Orthopedy and Travmatology in Medical Scholl at Fırat University in 1997-2004. They were treated conservative and sugical methods. Findings of last controlsof these patients were evaluated with present datas in their files and findings werecompared with datas of literature, improvement times, complications, advantages anddisadvantages of treatment methods were discussed and then some results wereremoved by literature datas.We studied seventy five patients who have tibia diaphysis fracture. Weapplied KR+UBSA to 22 patients, Ilizarov EF to 31 patients, intrameduller EnderNail to 8 patients, Orthofix to 4 patients, Locked Küntscher Nail to 8 patients, plaqueosteosyntesis to 1 patients and minimal osteosyntesis to 1 patient.We had these datas;Consensus hadn?t been arrwed for tibia diaphysis fracture treatments.Different treatment methods for tibia diaphysis fracture can be suggest.Conservative treatment by plaster of Tip A which is stabil or minimal shiftfracture is still the standart treatment method.Free reduction by plaque screw osteosyntesis mustn?t prefer out of eventswhich is adding and lengtiened fracture.Flexible intrameduller nails can be use for short oblic isthmus fracture withstrong fibula.First alternative of surgical treatment must be locked intrameduller nail whichis done with carving medulla.Intramedullar nail must prefer in Tip I and Tip II open fractures with carvingand Tip III A without carving.External fixations must be use in Tip III B, C fracture and reconstruction ofsoft tissue must do earlier results were had.In canclusion of statistical analysis, IM nailed Küntscher treatment has theshörtest union time with 16.4 week; whereas surgical treatment with EF has thelargest union time with 24.7 week.Key words: Tibia diaphysis fracture, conservative treatment, surgical treatment.2
