Abstract (EN)
Objective: COPD ranks fourth among the leading causes of death globally, and seventh in terms of disease burden. The usability of IMA and YKL-40 as biomarkers in the diagnosis and follow-up of COPD, their relationship with some markers of chronic inflammation, and their relationship with hypoxia and dyspnea were investigated. Method: In our prospective study, 40 COPD patients between the ages of 40-85 who came to our Chest Diseases Outpatient Clinic between January 2019 and January 2020 were included. As the control group, 39 people with similar demographic data, no other chronic diseases and no smoking were included. Results: Comparison of age, BMI, biomarker YKL-40 and IMA variables were made in the study groups. While the median value of IMA was found similar in the groups (p = 0.957), the mean age (p = 0.002) and YKL-40 value (p <0.001) were found to be higher in the COPD group. It was concluded that IMA does not have a diagnostic role, and YKL-40 has a diagnostic role in separating COPD patients (p = 0.001). The level of YKL-40 was lower in the control group compared to both stage 2 and stage 3 COPD patients (p <0.001, p = 0.017, respectively). Similarly, there was a statistically significant difference in CRP levels in the groups (p = 0.003). The CRP level in the control group was lower than in both stage 2 and stage 3 COPD patients (p = 0.001, p = 0.023, respectively), while stage 2 and stage 3 CRP levels were similar (p = 0.946). IMA, lactate and WBC levels were similar in the groups (p = 0.919, p = 0.543, p = 0.281, respectively). Conclusion: COPD is a chronic disease with high morbidity and mortality rates. It is important to make an accurate and early diagnosis of COPD. It was determined that further studies may be needed for IMA, which is in the content of our study, and YKL-40 can be used as a biomarker in the diagnosis and follow-up of COPD.
