Detection of O25B -ST131 clone of escherichia coli and CTX-M-15-like extended spectrum beta lactamase by PCR

Extended spectrum beta lactamases (GSBL)are common in Enterobacteriaceae, especially Escherichia coli and Klebsiella pneumoniae. a globally spreading international clone group multidrug resistance (MDR) especially characterized by E. coli serotype O25b:H4, phylogenetic group B2 and sequence type 131 (ST131)GSBL type CTX-M-15, making it difficult also provides the fluoroquinolone and aminoglycoside resistance infections developed multidrug In our study, it was aimed to determine the prevalence of ESBL positive 85 E. coli isolate ST131 clone and blaCTX-M-15 β-lactamase gene, as well as the antibiotic susceptibility status of urine samples taken from various service and outpatient clinic patients between 17.4.2019 and 21.7.2019. Bacterial identification, presence of ESBL and antibiotic susceptibility were defined with the VITEK2 automated system (bioMérieux, France). Sensitivity results were determined based on EUCAST criteria. ESBL production verified by double disc synergy test. Then, blaCTX-M-15 and blaCTX-M-3 genes encoding ESBL were analyzed by Real time PCR. Of a total of 85 ESBL positive E coli isolates, 59 (69.4%) were positive for the bla CTX-M genes, of which 25 (29.4%) were positive for CTX-M-15 and 34 (40%) were positive for CTX-M-3. , 26 (30.5%) of CTX-M-3 and CTX-M-15 did not have β-lactamase. For CTX-M-3 and CTX-M-15 positive 59 (69.4%) strains, the most sensitive (100%) carbapenem (ertapenem and imipenem) and the most resistant (100%) ampicillin and cephalosporin antibiotics were antibiotics. We detected ST131 clone by Real time PCR in 23 (27.1%) of 85 ESBL positive E. coli strains, while 62 (72.9%) were negative. In this group, the most sensitive antibiotics were 100% carbapenem (imipenem and ertapenem), while the most resistant (100%) were cephalosporins (cefixime, ceftazidime, ceftriaxone, cefuroxime), ampicillin and AMC.
