Plasma urotensin ii level and thiol-disulphide homeostasis in patients with fibromyalgia syndrome

Fibromyalgia syndrome (FMS) is a chronic musculoskeletal system disease which characterized by muscle aches, headache, fatigue, edema and sleep disorders. The etiology of fibromyalgia is still unclear. Urotensin II (U-II) peptid is the most potent vasoconstrictor known. It plays a role in the pathophysiology of many acute and chronic diseases. Li et al. have published a study in 2017 showed that U-II receptor antagonist alleviated neuropathic pain through the suppression of the JNK/NF-κB pathway. However, the clinical significance of UT II in FMS has not been studied yet. The rol of oxidative stress in FMS pathophysiology is controversial. Thiol/disulfide homeostasis is a very sensitive indicator of metabolic antioxidant/oxidant balance. Measurement of dynamic thiol/disulfide homeostasis was facilitated by the new automated method developed by Erel and Neselioğlu in 2014. In our study, it was aimed to evaluate plasma Urotensin II levels and thiol/disulphide homeostasis in order to contribute to elucidation of etiopathogenesis of FMS. 42 women who have newly diagnosed with (FMS) and have not started drug treatment yet followed up from Gaziantep University Şahinbey Research and Department of Physical Medicine & Rehabilitation, and age- and sex-matched 42 healthy individuals were included to our study for this purpose. Fibromyalgia impact questionnaire, hamilton depression and anxiety scales were collected from each patient. Urotensin II levels were determined by ELISA method. Thiol/disulphide homeostasis parameters were measured by autoanalyzer using the newly developed spectrophotometric method. Data obtained from the study were analyzed with the SPSS 24.0 package program. We found that in patients with FMS, urotensin II and disulphide levels, disulphide/native thiol and disulphide /total thiol percent ratios were higher in comparison with the control group, whereas total thiol and native thiol levels, native thiol/total thiol percent ratio were lower, but there was no statistically significant difference between the groups. As a result, our study findings showed that; To fully understand the role of U-II in the etiopathogenesis of FMS, especially based on central theories, there is a need for extensive human studies and new animal experiments. We also found that patients with FMS had an oxidative tendency, but we think that it is early to say that this non-significant height is one of the factors that play a primary role in the etiology of FMS.
