Özet (EN)

ABSTRACT BONE FORMATİON AND DEMOLİTİON MARKERS ABSERVED İN PATİENTS WİTH FİBROMYALGİA SYNDROME AND İT'S RELATİON TO OSTEOPOROSİS The aim of this study is to compare the effects of osteoporosis-related parameters with clinical, examination, functional and emotional evaluation variables. Therefore, it is aimed to investigate the effect of OP on disease severity and life quality in FMS patients and patients with low back pain who are in the control group. Fifty patients diagnosed with fibromyalgia syndrome (FMS) and fifty control patients diagnosed with degenerative low back pain were included in this study among prenemopausal patients with low back pain complaints. All patients' complete blood cell count, blood sedimentation values, serum CRP, RF, ALT, AST, ALP, CK, calcium, phosphorus, magnesium, parathormone, vitamin D, TSH values, Bone remodeling markers (Bone Alkaline Phosphatase, Osteocalcin), Bone destruction markers (hydroxy proline, type I collagen carboxy terminal cross-link telopeptide β-CTX), lomber and hip DEXA values in this research are reviewed. All patients' tender points are examined by pressure algometry and digital palpation. Fibromyalgia impact questionnaire (FIQ), hospital anxiety and depression scale (HAD). Fatigue severity scale (FSS), modified fatigue impact scale (MFIS), Nottingham health profile (NHP), Oswestry scale and Qualeffo-41 questionnaires were implemented for functional, emotional and general health evaluations of the patients. Fibromyalgia Impact Questionnaire (FIQ), HAD, FSS, MYES, NSP, Oswestry scale and Qualeffo-41 scores were significantly higher in the FMS group than the control group (p<0.05). No statistically significant difference has been found Bone Alkaline Phosphatase, osteocalcin, hydroxyproline and CTX values between the control group and the FMS group. (p>0.05) In the FMS group, DEXA measurement values (BMD and T score) are statistically significantly lower than the control group (p<0.05). Correlation has been found between Vitamin D, CTX values and MFIS, FIQ, doctor's global VAS assessment, patient's global VAS assessment, Parathormon, Oswestry, Qualeffo-41 scores (p<0.05). Among the all patients in the research, the patients in the FMS group are more prone to osteoporosis. The reason for the FMS patients' osteoporosis predisposition is that they have worse functional, emotional and general health profile. For these patients, osteoporosis is a major cause of morbidity and should not be disregarded. Keywords: Fibromyalgia Syndrome, Osteoporosis, prenemopausal, Bone Densitometer
