Abstract (EN)

Introduction and Purpose: In recent years, it has started to operate low fresh gas flow anesthesia control with the use of modern anesthesia devices with advanced monitoring and safety features. Low fresh gas flow anesthesia during inhalation anesthesia; It has less negative effects on the pulmonary system, ecological balance and economy compared to normal flow fresh gas anesthesia. It has been observed that it affects mucociliary clearance, heat and fluid loss less. It can save up to 75% by reducing the consumption of anesthetic gas. In our literature review, there were limited studies comparing the effects of low fresh gas flow desflurane anesthesia and normal fresh gas flow desflurane anesthesia on hemodynamics, blood gas parameters and recovery in patients undergoing laparoscopy surgery. The aim of our study is to compare the effects of low-flow desflurane anesthesia and normal-flow desflurane anesthesia on intraoperative hemodynamics, blood gas and postoperative recovery. Material Method: Sixty patients, aged between 18 and 70, who were in the ASA II-III class, who were administered general anesthesia for two hours or more were included in this study. The patients were randomly divided into two groups of 30 each. I. group Low flow Desflurane Group (n=30), II. group was named as Normal flow Desflurane Group (n=30). TGA: 2L/min (FiO2: 50%, Air: 50%) in Group II under normal flow anesthesia, TGA: 0.5L/min (FiO2:70, Air: 30%) in Group I under low flow anesthesia ) was determined. HR, MAP, and SpO2 values were recorded in the preoperative period, immediately after intubation and at the 15th, 30th, 45th, 60th and 120th minutes following intubation. Arterial blood gas values were recorded in the first 60-120 minutes of the operation. After the operation, the patients were taken to the postoperative care unit and the modified aldrete recovery scale, respiratory and hemodynamic parameters were followed for 60 minutes, and they were transferred to their clinics without any problems. Findings: There was no significant difference in the blood gas parameters measured at the 1st and 2nd hour intraoperatively in the low flow anesthesia and normal flow anesthesia group. There was no significant difference between the groups in terms of intraoperative hemodynamics and postoperative recovery criteria. In the normal flow group, PaO2 was found to be statistically significantly lower in the smoking group compared to the non-smoker group (p<0.05). Conclusion: In conclusion, no significant difference was found in the low flow desflurane group in terms of hemodynamics, intraoperative blood gas parameters and postoperative recovery criteria in our study. We found that PaO2 was statistically significantly lower in the smoking group in the normal flow group than in the non-smoker group.
