The effect of the colchicine on the fibrotic process after the injury of the acute uretra in the animal model

Introduction and Objective: The definition of urethral stricture covers the anterior urethra and is an abnormal narrowing of the urethra caused by fibrosis in the peripheral corpus spongiosum. The main factor that prepares the ground for the disease development process is the formation of fibrosis during recovery. Various surgical and medical treatments are available to keep the urethral lumen open and prevent this fibrosis. In our study, we planned to investigate the effect of colchicine with antifibrotic activity on the development of fibrosis during healing in the traumatized rat urethra. Material and Methods: In our study, 20 male Wistar-Albino rats weighing 250-300 grams were used. First, a healthy rat was taken and the urethra of the rat was traumatized with the help of a branule needle, and the urethra and bladder were excised as a whole. In the dissection made to the specimen, a scar including the corpus spongiosum of about 2 cm in length was observed in ventrale side of the urethra. The remaining 19 rats were divided into three groups. No intervention was taken on the two rats in the control group; The urethra of seven rats in the sham group was traumatized at 12 o'clock and no additional treatment was given; after the urethra of 12 rats in the study group was traumatized, oral colchicine was started at a dose of 1 mg/kg/day and all rats were sacrificed on the 28th day. The urethra was excised and the traumatized area proximal A was named distal B location. Urethral specimens fibrosis formation, mononuclear cell infiltration and vascular proliferation were evaluated histopathologically. Results: For the A location, all three groups were compared in terms of fibrosis, mononuclear cell infiltration and vascular proliferation. Considering the results, the traumatization procedure made a statistically significant difference in two parameters in location A (fibrosis p = 0.013 and vascular proliferation p = 0.02). However, this statistically significant difference was not found at location B and by combining both locations. Then, the study and sham group were compared according to A and B localizations in terms of colchicine efficacy. A statistically significant difference was obtained for sham and study group comparisons for location A for fibrosis (p = 0.037) and vascular proliferation (p = 0.048). The difference here is that the sham group scores are low. There was no significant difference in all three parameters for location B and by combining both locations. Conclusion: Our study consists of two arms investigating experimental urethral traumatization formation and the effect of colchicine on urethral spongiofibrosis. Traumatization performed with branule needle in the first step has produced a statistically significant effect even if it is not in all parameters and comparable results have been obtained with other methods in the literature as an effective method for generating experimental spongiofibrosis in the rat urethra. In our study, it was revealed that colchicine had no preventive effect on the development of spongiofibrosis after urethra traumatization. Studies to be planned with more homogeneous groups and well-adjusted colchicine dose ranges may contribute to understanding the effect of the drug on urethral stenosis.
