Influence of animal origin bone graft material (Bioteck®S.R.L. Osteoplant®-Flex System) on the stability of orthognatgic surgery

The aim of orthognathic surgery is to correct a dental malocclusion and facial deformity. The Le Fort I osteotomy is the most common maxillary procedure for correction of dentofacial deformities. The restoration of normal jaw funtion, the creation of optimal facial aesthetics and the maintenance of long-term stability are considered the sine qua non of successful orthognathic surgery.Bone grafting is know to improve stability after midface osteotomy. The obvius disadvantages of autogenous bone grafts are the related donor site morbidity and postoperative bone graft resorption which may lead to osseous instability.This study retrospectively investigated the long-term stability of Le Fort I osteotomy after maxillary impaction and\or advancement with use of animal origin bone grafting. We analyzed radiographic data on 17 patients with maxillary deficiency. Presurgical, postsurgical and one-year after postsurgical cephalometric radiographs were evaluated. The magnitude of relapse after one postoperative year and relapse ratio were analysed. Relapses were compared with the literature.We determined that, the most stable movement was maxillary impaction (5,1 % relapse) and relapse after maxillary advancement and inferior repositioning was low (7 % and 8,7 % relapse). We observed the minor complication.Consequently, the Le Fort I osteotomy is frequently used for the correction of dentofacial deformities. Rigid internal fixation combined with bone grafting may improve stability of maxillary inferior and\or anterior repositioning after Le Fort I osteotomy. However autogenous bone grafts have some disadvantages. Maxillary inferior and\or anterior repositioning with Le Fort I osteotomies by using rigid fixation and interpositional animal origin bone graft material is a stable and predictable procedure.Key Words: Le Fort I osteotomy, maxillary repositioning stability, animal origin bone graft material,
