Evaluation of epidemiologic, laboratory and clinical properties of patients with henoch schönlein purpura

Abstract (EN)

Henoch Schönlein Purpura (HSP) is the most common small vascular vasculitis of childhood. It can hold all systems, especially the skin, joints, gastrointestinal system and urinary system. Non-glycolyzed IgA1 plays a role in its pathogenesis. Although its etiology is not yet clear, upper respiratory tract infections and drugs are thought to trigger the disease. In this study, it is aimed to evaluate the relationship between laboratory and clinical findings by examining the epidemiological, clinical features and laboratory findings of patients diagnosed with HSP. 120 patients were included in the study. In these patients, the female to male ratio was 1/1. The average age of the patients was 7.5 ± 3.4 and the most common season was autumn (32%). Half of the patients (50%) had upper respiratory tract infection and 38.3% had a history of β lactam antibiotics. 106 (88.3%) of the 120 patients examined had a rash at the time of first admission joint involvement was detected in 77 patients (64.2%) and gastrointestinal system involvement in 63 patients (52.5%). Central nervous system involvement was present in only 1 patient (0.08%). The white blood cell values of the patients are between 5.000-37.000; Increased CRP in 79 patients (73%), leukocytosis in 84 patients (71.1%), and thrombocytosis in 26 patients (22%). C3-C4 level was found to be low in 3 patients (10%) compared to age. USG was performed in 70 patients who participated in the study, 13 (10.8%) of these patients had mesenteric lymphadenitis and 5 (4.2%) of intussusception. Renal involvement was detected in 31 patients (25.8%). 16 people (13.3%) with microscopic hematuria were detected. Of the 120 patients included in the study, 91 (75.8%) received inpatient treatment. The most commonly used non-steroidal anti-inflammatory drug ibuprofen (72.6%) in the treatment. Corticosteroid use rate was found to be 39.1%. In conclusion, it is obtained that, whole patients in our study with HSP, HSP was self-limited. Despite all treatments, relapse could not be prevented and recurrence was observed at 9.8%. Long-term follow-up of the disease is important due to possible complications and recurrence outside the acute period.
