Abstract (EN)
ABSTRACT Objective: The aim of this study was to determine whether there was a difference in the rate of glycemic control among patients who had frequent blood glucose measurements and those who had less blood glucose measurements at home in the period following discharge after diabetes and nutritional education and blood glucose control were given to the hospitalized patients due to high blood glucose levels. Method: This research, Akdeniz University Hospital of Endocrinology and Metabolic Diseases, inpatient treatment Clinic, insulin fits into use and discharge until they are followed by research criteria is randomly selected 100 (Type 1 and Type 2) was performed on the patient. When group 1(frequent measurement) was discharged from the hospital; 1.week, 2.week, 3.week and 4 th week, Group 2(less measurement), 1.Week and 4th week, one a week 7-point capillary blood glucose measurements were formed by patients. Analyzes were performed with SPSS 22.0 software package and p <0.05 was considered statistically significant. Results: 55% of the patients only use insulin, 45% of them use oral antidiabetic (OAD) and insulin. The fasting blood glucose averages were similar in both groups (p = 0.268). After discharge, worsening of 7-point blood glucose levels was observed in both groups (p <0.05). The mean glucose of patients in Group 1 at the 4th week after discharge was 150,80 ± 59,88 mg / dl, and in group 2, 199,62 ± 81,27 mg / dl (p = 0.002). In Group 2, there was a 0.51% decrease in HbA1c after 1 year of hospitalization (p> 0.05) and a decrease of 1.04% in patients in Group 1 (p<0.001). Conclusion: Frequent blood glucose measurements at home and contact with patients by phone can improve glycemic control by contributing to patients' self-management.
