Our results of endonasal endoscopic surgery in patients with idiopathic cerebrospinal fluidrhinorrhea

Aim: In this study, we aimed to evaluate the efficacy of endoscopic endonasal surgery in patients with idiopathic cerebrospinal fluid (CSF) rhinorrhea. Materials and Methods:In this study, endonasal endoscopic surgery for idiopathic CSF rhinorrhea was performed retrospectively between January 2009 and May 2018 at Balcalı Hospital of Çukurova University Medical Faculty. Pediatric patients, trauma, tumor, or previous cranial and endoscopic endonasal surgery were excluded from the study. Demographic characteristics, defect localization, side, size, radiological features, graft / flap materials used, additional treatment methods and complications were analyzed in total 24 cases. Results:Of the 24 patients included in the study, 16 were female, 8 were male and the mean age was 52.8 ± 12.0. When the defect localization was examined, 33.3% was the most common sphenoid sinus lateral wall defect and the second was 25.0% of olfactory sulkus region, with 58.3% on the right side, 37.5% on the left side. On radiological examination, more than half of the patients (54,2%) were Keros type 2. Defect size ranged from at least 1 mm to at most 2 cm. Postoperative hospital stay was between 6 days and 60 days. 11 of the patients had lomber drainage. Most fascia lata (66,6%) were used to close the defect in the surgeon. Complications encountered were meningitis, pnömokokal sepsis and mortality in a patient. 1 case was treated with revision surgery due to recurrent rhinorrhea and craniotomy was continued after complaints continued.Bilateral defect was observed in 1 patient. Conclusion: Despite the difficulties in diagnostic methods, idiopathic CSF rhinorrhea can be treated minimally invasively with most defects endoscopically in medical technology and surgical techniques. Contrary to external approaches, endonasal endoscopic method with low morbidity and mortality is an effective treatment method. Key words:Endoscopic sinus surgery, idiopathic, rhinorrhea.
