Retrospective evaluation of patients who admitted to our department with drug reaction

Abstract (EN)
In this retrospective study, We investigated the results of 68 patients who admitted to the, Department of Child Health and Diseases, Discipline of Child Allergy and Immunology because of drug reaction between August 2015 and September 2018 in University of Dokuz Eylül. Drug allergy was defined in only 11 patients among the 68 patients. When we evaluated the patients with suspected drug allergy in terms of atopy (atopic dermatitis, urticaria, allergic rhinitis, nutrient / inhaler allergy, asthma); 33 (48.5%) patients had no atopy and 35 (51.5%) patients had a history of atopy. We analyzed the eosinophil levels (mm3 / cell) of 62 patients of 68 patients who applied with suspicious of drug allergy. We detected the 295.48 mm3/cell average level of eosinophil. When we divided these patients as a allergy reaction positive and negative, we observed that Average level of eosinophil in 52 patients with allergy reaction negative and 10 patients with allergy reaction positive was 325.38 mm3/cell and 140 mm3/cell, respectively. We compared the patients in terms of total IgE and eosinophil levels, taking into account atopia in the clinical history of our patients. The average of total IgE levels in the patients with atopy was 275 IU/mL and the average of total IgE levels in the patients with no atopy was 167 IU/mL We focused on eosinophil levels of patients we grouped in terms of atopy; the eosinophil average level of patients with no atopy was 314 mm3/cell and those of patients with atopy was 279 mm3/cell 12 reactions(100%) were determined in 11 patients in terms of the time interval between the application of the last dose of the drug responsible for the reactions and the occurrence of the reaction. While 9 reactions(75%) of them were determined in first 1 hours (sudden), 3 reactions of them (25%) were determined in 1-72 hours(rapid). But no patient was observed delayed-type reactions All of 68 patients who apply with suspicious of drug reaction were evaluated by provocation test against different drug ingredients. beta-lactam antibiotics were determined as the most common drug causing the reactions. Amoxicillin clavulanate among beta lactam antibiotics was determined as the most responsible ingredient. As data about morbidity, mortality and resulting economic loss that drug reaction causes in children increase, necessity of studies on the subject is beter understood. Determination of epidemiological and demographic characteristics in children can provide beneficial information about the irpreventability as well as the approach to be followed in these reactions.
