Abstract (EN)
Objectives: Thromboembolism is common in cancer patients. Prophylaxis and treatment of thromboembolism in patients with cancer increase survival. Several molecules of the coagulation and fibrinolysis are activated during development and progression of colorectal cancer. TAFI, TFPI, prothrombin fragment 1+2 define novel moleculer connections between blood coagülation and fibrinolysis. The aim of the present study is to identify the relationship between patient characteristics and levels of blood TAFI, TFPI, F1+2 in advanced colorectal cancer patients.Method:. Patients with advanced colorectal cancer were enrolled and TAFI, TFPI, F 1+2 were assessed by enzym linked immunabsorbant assay. Patients with history of thromboembolic event, use of drugsthat affects coagulation and those with central intravenosous device were excluded. Patients characteristics such as age, sex, performance status, body mass index, time to progression, number of metastasis, tumor size, localisation, vascular invasion, perineural invasion, CEA, hemoglobin, hematocrit and chemotherapeutics received were retrospectively reviewed. The relationship of patient characteristics and TAFI, TFPI, F 1+2 levels were assessed by NCSS 2007&PASS 2008 statistical software.Findings: Twenty eight women(thirty four percent) and fifty four male patients(sixty six percent) participated in the study. The mean age of the patients was 56. TAFI, TFPI and prothrombin fragment 1+2 levels of the patients were observed as high respectively %69. 5, %71 and %96. 3. When patients? characteristics was compared it was seen that there was a negative relation between age and TFPI though it wasn?t significant statistically. TAFI levels were higher in female patiens compared to men yet it wasn?t meaningful statistically . In patients with performance score ECOG 2 F 1+2 levels were statistically significantly high. Patients with PKGS over 10 months had high TFPI levels though they weren?t significant statistically. In rectum cancer diagnosed patients TAFI levels were statistically significantly high. In patients with grade 2,3 tumor TFPI levels were significantly high. There was a statistically unsignificant weak correlation in a positive direction between CEA and hemoglobine values and TFPI. A positive relation was observed among platelet and TAFI levels yet it wasn?t statistically significant in a positive way. No connection was found between number of metastasis, size of the tumor , vascular invasion, perineural invasion and treatment protocols with TAFI, TFPI, F 1+2 levels in our study.Conclusion: Changes in hemostatic system and chronic activation are frequently observed in cancer patients. TE is a leading cause of death among patients among patients cancer and contributed to long term mortality in patients with early and advanced stage cancer. The pathophysiology of the association between thrombosis and cancer is complex; malignancy is associated with a baseline hypercoagulable state due to many factors. TE in a cancer is a multifactorial dissease. Stratification of cancer patients according to their TE risk is of the utmost clinical importance, since patients at the high risk TE may benefit from thromboprophylaxis. A risk scor system has not yet been discovered. Several recent studies have reported that stratification of cancer patients according to their TE risk can be accomplished by measuring biomarkers. Any molecule has not been yet validated as a biomarker.Key Words: Cancer, thrombosis, TAFI, TFPI, prothrombin fragment 1+2.
