Evaluation of pregnancy results of patients which WAS applied laparoscopic surgery due to infertility

Purpose: It was aimed to retrospectively evaluate the pregnancy outcomes of patients who underwent laparoscopic surgery for infertility. Materials and Methods: The material of our study consists of 58 patients who underwent L/S-guided adhesiolysis, chromotubation and laparoscopic surgery (cystectomy, cyst extirpation, neosalpingostomy etc.) with the diagnosis of primary or secondary infertility between 01 January 2017 and 30 September 2021 at the Dicle University Medical Faculty Hospital Gynecology and Obstetrics Clinic. The L/S procedure was performed by the same surgeons who are experts in the field of infertility. First of all, all patients underwent laparoscopic observation. Afterwards, chromatubation was performed by injecting methylene blue into the uterine cavity via a uterine manipulator and the findings were noted. Patients who were subsequently found to have intracavitary pathology were excluded from the study. Laparoscopic surgery was performed in patients with pelvic adhesions, hydrosalpinx, ovarian cysts or tubal obstruction. Results: Thirty-four (58.6%) of the patients were found to be primary infertility, and 24 (41.4%) of them were secondary infertility. According to the previous gynecological surgery history of the patients, 11 of the 58 patients (19%) had a history of previous surgery. The number of patients who underwent tubal surgery and adhesiolysis was 27 (46.5%). The number of patients who underwent ovarian surgery (cystectomy, cyst extirpation) was 14 (24.1%). In the follow-up of the patients after laparoscopic surgery, pregnancy was achieved in 19 (32.8%) of 58 patients, and pregnancy could not be achieved in 39. The median value of the time until pregnancy after the surgical procedure was calculated as 9 months (min max value: 4-24 months). Of the 19 patients who achieved pregnancy, 3 achieved pregnancy with IVF, 3 with IUI and 13 without any procedure. Of the 17 patients who had a live birth, 6 had normal vaginal delivery and 11 had cesarean section. Conclusion: We think that the correction of pelvic pathologies (adhesions, tubal and ovarian pathologies) that we frequently detect in patients undergoing laparoscopic surgery for infertility contributes positively to post-operative pregnancy outcomes.
