Determination of psychological insulin resistance and self-care in patients with type 2 diabetes using and not using insulin

Abstract (EN)

Objective: This study was conducted to determine psychological insulin resistance and self-care in type 2 diabetic patients with and without insulin use. Material and Method: The study was descriptive and correlational and the population consisted of type 2 diabetes patients who were being treated in the internal medicine clinics of a state hospital in a province. In the study, no sample selection was made, 200 type 2 diabetes patients with criteria suitable for the purpose of the research were taken on the basis of volunteerism. Descriptive characteristics form, Psychological Insulin Resistance Scale (PIDS) and Diabetes Self-Care Scale (DSS) were used to collect the data. Results: It was determined that 79% of the patients participated in the study were female, 39% were in the age range of 51-60 years, 97.5% were married and 83% were illiterate. The mean score of the patients was found to be 37.52±13.79 and the mean score of the patients was found to be 80.42±19.60. The mean score of the patients who used insulin was found to be 24.11±5.14 and the mean score of the patients who did not use insulin was found to be 48.06±8.10 and the mean score of the patients who did not use insulin was found to be 86.83±22.39. Among the patients who used insulin, it was determined that the PID was lower in patients who were primary school graduates, unemployed, smokers, had additional chronic diseases, had a disease duration of 10 years or more, did not receive diabetes education and were overweight. Among the patients who did not use insulin, it was determined that patients who were male, under 50 years of age, illiterate, single, smokers, did not drink alcohol, had a diagnosis of diabetes for 1-3 years, had no other diabetic patients at home, and did not receive education had lower PID. It was determined that patients aged 51-60 years, primary and secondary school graduates, with middle income level, without additional chronic disease, with a diagnosis of diabetes for 7-9 years, receiving diabetes education, and thin patients who used insulin had higher levels of PID. In patients who did not use insulin, patients under 40 years of age, with high school and higher education level, married, with high income, non-smokers, with a disease diagnosis of more than 4 years, with no other diabetic patients at home were found to have higher levels of SLB. Conclusion: As a result, it was found that the PID levels of insulin-using patients were moderate and their self-care was below the acceptable level. It was determined that education, employment status, smoking, education and BMI parameters affected PID. It was determined that education, income, additional chronic disease and duration of disease diagnosis affected the level of SLD in patients using insulin.
