Comparison of eye medical evidence and ophtalmic artery doppler parameters with the functional capacity of patient in heart failure

Abstract (EN)

AIM: In heart failure insufficiency of circulation and congetion decrease the life standart functional capacity and exertional tolerance and also damages all organs fuctions. İn this study we want to show the effect of worsening the functional capacity on eye medical evidence and dopler parameters of oftalmic arterMATERİAL AND METODS: We separate the patients that are ages between 23-84 in sinus ritm according to the NHYA functional capacity. It?s prefered in this study three groups instead of four classes of NYHA. Group 1; includes 28 patients who are Class I, Group 2 ; includes 32 patients are Class II. Group 3 ; includes 31 patients are Class III -IV.Patients blood cell,biochemistry EKG are checked and than their EF, FS ,BMI are calculated. Patients are exemined at Dicle University Oftalmology Clinic, their visual acuity, corneal thickeness by pakimetry, eye pressure by goldman aplanasyon tonometer and fundus findings by biomicroscop are studied and evidence of hipertansif retinopaty, diabetic retinopaty, retinal atropy and results of oftalmic arter doppler ultrason are wrote down and with all this findings statistical analizes is made by using SPSS 11.5 program and Krusskal-Wallis analise.RESULTS: Patients blood cell, lipid profil, BMI and risc factors for heart faiulare are nearly the same in all groups. In this study we analyise that when functional capacity got worse,urea (p:0,006), total biluribin (p:0,001), direct biluribin (p:0,003) increased and EF, FS paremeters (p:0,004-0,003) of patients and the level of Na (p:0,03) decreased. There wasn?t significant difference between corneal thickness right eye (p:0,179), left eye (p:0,118) and eye pressure right eye (p:0,161), left eye (p:0,111) and fundus findings in the groups. There was not significant decreasion in left peak sistolic velocity (p:0,055) and left end diastolic velocity (p:0,087). There was not significant decreasion in right end diastolic velocity (p:0,068) and right peak sistolic velocity (p:0,071), There was no relationsheap between right resistivity index (p:0,635), left resistivity index (p:0,635) and right pulsatil (p:0,617), left pulsatil index (p:0,455). However worsening functional capacity decreased the oftalmic arter velocity paremeters.CONCLUSION: In this study worsening functional capacity demaged the liver and kidney founctions and no significant decreased the velocity and perfusion of the oftalmic arter but it did not influence the eye exemine evidence.KEY WORDS:Heart failure, functional capacity, ophtalmic doppler ultrasound, ophtalmic artery
