Evaluation of Common Cerebral Edem role in pathogenesis of Idiopatic Intracranial Hypertension by using Quantitative Diffusion MRI values

Abstract (EN)

AIM: In this study, we sought to evaluate the relationship between diffuse swelling of the brain in the pathogenesis of patients with idiopathic intracranial hypertension (IIH), which corresponded to the revised criteria Dandy, using measurements of the diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC). MATERIALS AND METHODS: Patients with a diagnosis of intracranial hypertension who met the revised Dandy criteria for brain MRI at Dicle University Medical Faculty Radiology Department between 01.01.2016 and 01.01.2019 were included in the study. The digital archive linked to the PACS (picture archiving communication systems) system was retrospectively scanned.ADC measurements were performed at 24 different points (bilateral frontal and parietal cortex, insula, cingulate gyrus, hippocampus, frontal and occipitoparietal periventricular white matter, caudate nucleus, putamen, thalamus, cerebellar hemispheres, pons, and mesencephalon) in the right and left cerebral hemispheres in all patients with a diagnosis of IHD with diffusion-weighted MRI. In this study, SPSS18.0 was used to compare the mean ADC values obtained from different parts of the brain parenchyma of IHD patients and control groups. Values with a p value less than 0.05 were considered statistically significant. RESULTS: In our study, the patient group and the healthy control group consisted of a total of 90 patients (39 women and 6 men in each group).The mean age distribution was 38.07 ± 6.4 in the patient group and 38.32 ± 6.4 in the healthy control group. Statistically, there was no significant difference in age or sex distribution between the patients with IIH and the control group (p = 0.73; p = 1,000, respectively).There was no parenchymal abnormality when conventional MRI and DWI sequences were evaluated in the control and IIH patients.There was no statistically significant difference between the mean ADC values of 24 different regions of the brain between 45 healthy control groups and 45 patients (p> 0.05). CONCLUSİON: In our study, no significant difference was found between the mean ADC values obtained from certain regions of the brain and the values obtained from normal healthy controls. We think that brain edema does not contribute to the pathogenesis of IIH.
