Comparison of Conventional Open Surgical Vein Harvesting and Minimally İnvasive Endoscopic Vein Harvesting about Problems İn Wound Healing and Patient Satisfaction

Harvesting saphenous vein with conventional methods brings some problems including pain, redness, wound drainage, dehiscence, need to open the superficial incision, abscess and patient?s unsatisfactoriness. In this study problems come with conventional saphenous vein harvesting aimed to be compared with endoscopic procedure. One of the method developing to solve the problems come with harvestig saphenous vein with traditional open surgical method is minimally invasive endoscopic procedure. The aim of this study is to compare two techniques about wound healing disturbances.42 patients have been operated for coronary artery revascularisation and used saphenous vein like otogen graft in Abant İzzet Baysal University Medical Faculty beetween January and December 2009 investigated prospectively.Patients undergoing endoscopic procedure was named group one (n=15) and conventional procedure group 2 (n=27).At the 3, 5, 15, 30 and 60th days after operation, patients investigated about hematoma, superficial ecchymosis, redness, purulent discharge, dehiscence, pain, abscess on direct examination, early or late deaths, heat plus the need to open the superficial incision and satisfaction.There was statistical significant difference beetwen two groups about redness (at the 15 and 30. days p=0,031; p=0,049), purulent discharge (at the 3,15 and 30. days (p=0,049; p=0,007; p=0,031), dehiscence (at the 15, 30 and 60 days=0,049; p=0,031; p=0,049) in favour of endoscopic group. When satisfaction was evaluated statistically, significiant difference also appeared in favour of endoscopic group (at the 15 and 30 days p=0,042; p=0,026)There was significiant difference in favour of endoscopic group about patient satisfactionOnly superficial ecchymosis was in favour of conventional group at first 5 days which brings no addiotinal inconvenience.%93 of patient in endoscopic group has had no problem about wound healing, whereas only %33 of patients experienced problem free healing in traditional group.In conclusion, this study has shown efficacy, less wound healing problems and much superior cosmetic results of minimally invasive endoscopic procedure. Long term patency rates of conduits harvested with endoscopic procedure need to be followed.
