Nerve growth factor (NGF) levels in chemotherapy-induced peripheral neuropathy (CİPN)

AIM: Chemotherapy-induced peripheral neuropathy (CIPN) is a common complication caused by neurotoxic drugs. This study investigated whether nerve growth factor (NGF) could be a predictive biomarker for CIPN in children undergoing cancer treatment. MATERIALS AND METHODS: The study involved 28 pediatric cancer patients treated with neurotoxic chemotherapy at Sakarya University. NGF levels were measured one day before, one week after, and one month after chemotherapy. The relationship between NGF levels and neuropathy development was analyzed. RESULTS: Motor neuropathy was observed in 28.57% of the patients, all treated with vincristine, and their symptoms improved post-treatment. NGF levels increased from 312.25±195.59 on day zero to 393.84±165.38 on day seven and 408.84±203.8 on day thirty. However, there was no significant correlation between NGF levels and motor neuropathy. Patients with motor neuropathy received lower vincristine doses, though cumulative dosage was not linked to neurotoxicity, which tended to develop early in treatment. The lack of a sustained NGF level increase was associated with higher neuropathy risk. Logistic regression and ROC analyses did not identify predictive markers for motor neuropathy. CONCLUSION: Neuropathy occurred in one-third of children treated with vincristine, highlighting the importance of detailed neurological assessments during chemotherapy. However, there was no direct link between NGF levels and neuropathy; NGF's potential neuroprotective role warrants further research. Comprehensive monitoring is recommended for all children receiving vincristine or similar drugs.
