Abstract (EN)
Objective: We aimed to compare the operation and anesthesia types, evaluate the postoperative hospitalization times, preoperative and postoperative hemoblobin and hematocrit values, using drain and complications of the patients who were clinically diagnosed and operated with leiomyoma. Material and Methods: This study includes 378 patients who were diagnosed with leiomyoma between January 2014 and December 2018 at Dicle University Medical Faculty Obstetrics and Gynecology Department. Demographic characteristics (age, gravida, parity, hospitalization days, leiomyoma number, leiomyoma size and drain), laboratory tests (hemoglobin and hematocrit levels) and surgical informations were obtained from hospital archive files and archive database. Descriptive statistics, chi-square and student t tests were used for the analysis of the data by SPSS 22.0 package program and p <0,05 values were considered significant. Results: 169 (44.7%) patients performed myomectomy by laparotomy, 156 (41.3%) total abdominal hysterectomy, 15 (4%) myomectomy by L / S, 13 (3.4%) TLH(Total Laparoscopic Hysterectomy), 12 (3.2%) myomectomy during cesarean section, 9 (2.4%) underwent operative hysteroscopy and 4 (1.1%) vaginal myomectomy. The mean age was 46.8 in total abdominal hysterectomy and 36.7 in myomectomy. The most frequently complaint of the patients was bleeding disorders (164 patients %43.5), and the second was pain (140 patients %37). The postoperative result of pathologies, 341 (88.2% ) was leiomyoma uteri, 32 (8.4%) was leiomyoma + adenomyosis and 13 (3.4%) was STUMP(Smooth Muscle Tumors of Uncertain Malignant Potential). At the postoperative period infection developed at the 1 patients incision line. 5 patients' bowel and 1 patient's bladder injured during hysterectomy. Conclusion: Uterine sparing surgery was preferred especially in young patients. Complications may be higher even in abdominal surgery. With the development of laparoscopic and robotic surgery, the importance of more minimally invasive surgery is gradually increasing. Further prospective studies are needed to compare this. Keywords: Leiomyoma uteri, hysterectomy, myomectomy, hysteroscopy
