Abstract (EN)

Mitral stenosis is the most common rheumatic heart valve disease. Although there are various reasons in the etiology, rheumatic fever is the most common cause. Rheumatic heart disease is an important health problem in developing countries. Inoue et al. described the percutaneous mitral balloon valvuloplasty (PMBV) in 1984 and this procedure replaced surgery in most of the patients with mitral stenosis. Nowadays most of the patients with symptomatic mitral stenosis and appropriate valve morphology undergo PMBV procedure. Studies showed this procedure has good short and long term outcome. In this study, we aimed to evaluate early outcomes of percutaneous mitral balloon valvuloplasty cases in our clinic. 52 patients (36 females, 16 males, meanage 48 ± 13 years) with severe mitral stenosis, symptomatic moderate mitral stenosis and echo score ≤ 12 are included in this study. In our study, mitral valve are increased from 1.18 ± 0.19 'cm² to 2.27 ± 0.48 cm² (p = 0.001), the mean mitral gradient decreased from 12.82 ± 3.5 mmHg to 5.97 ± 2.04 mmHg(p = 0.001) with valvuloplasty. Significant reductions were also achieved in other hemodynamic parameters. PMBV was successfully applied in 87% of our patients, 90% with an echo score ≤ 8, and 80% in the group with an echo score of > 8. The procedure was unsuccessful in 7 patients; 3 patients in the group with echo score ≤ 8 and in 4 patients in the group with echo score > 8. The procedure was successful in 2 pregnant patients. In failed cases, procedure MVA was significantly lower. Although PMBV success was low in the group with high echo score, statistical significance was not maintained (p = 0.02). The reasons of our failed cases were the failure of valve in two patients and failure of septostomy in one patient, so PMBV could not performed. In two cases, there was less opening (<1.5cm²) in the valve. As a result, PMBV can be performed in patients with echo score 9-12 and may make hemodynamic and symptomatic improvements as in those with symptomatic mitral stenosis, echo score ≤ 8.
