The relationship between the stage, matrix metalloproteinase and metalloproteinase tissue inhibitors and the relationship between podocite and tubular damage markers in chronic renal failure patients

Objective: Chronic kidney disease (CKD) is an important public health problem in the world and in our country. According to current data, the increase in the prevalence of CKD continues and its rate of increase is decreasing. In our study, we evaluated the relationship between endothelial dysfunction markers and tubular and glomerular damage and their biochemical markers according to the stages of CKD in patients who developed CKD due to various etiological reasons. We investigated the etiology of CKD and the effect of the stage of the disease and endothelial damage on kidney functions. Materials and methods: The study was designed as a prospective case-control study. Applied to Düzce University Medical Faculty Hospital Nephrology and Internal Diseases services and outpatient clinics between January 2020 and June 2020, 132 patients with a diagnosis of stage 1,2,3,4,5 CKD with a diagnosis of CKD according to KDIGO criteria and a control group of 38 people were included. In the patient group, 23 patients were stage 1, 22 patients were stage 2, 23 patients were stage 3a, 20 patients were stage 3b, 23 patients were stage 4, 21 patients were stage 5 CKD. Information such as age, gender, comorbidities, medications used, and blood and urine samples were taken from the patient and control groups. Kidney Injury Molecule-1 (KIM-1), Matrix metalloproteinase (MMP) -2, MMP-9, Tissue inhibitors of metalloproteinase-1 (TIMP), TIMP-2 levels from blood samples of both groups and nephrine levels in urine samples ELISA It was examined by method. Results: Our study included a total of 170 people, 132 patients and 38 control group. All analyzes were made by correcting for age. As the CKD stage increased, plasma KIM-1, TIMP-2, MMP-2 and urine nephrine levels increased (p <0.001, p <0.001, p <0.001, p <0.05, respectively). In the group using calcium channel blocker, plasma MMP-2 levels were found to be statistically significantly lower (p <0.05). Conclusion: Regardless of the etiology of CKD, we think that plasma KIM-1, MMP-2, MMP-9, TIMP-1, TIMP-2 and urinary nephrine levels can be used as an indicator of renal damage.
