Investigation of the frequency of liver and pancreatic steatosis in patients with chronic hepatitis B, chronic hepatitis C and Wilson's disease

Objective: We planned to analyze the relationship of chronic hepatitis B, chronic hepatitis C and Wilson patient treated in our center with pancreatic and hepatic steatosis and present it as a center's experience Materials and Methods:In our study, patients diagnosed with non-cirrhotic Wilson's Disease, chronic hepatitis B, and chronic hepatitis C followed in Gaziantep University Şahinbey Research and Practice Hospital Hepatology outpatient clinic were included. 3660 patients followed in the hepatology outpatient clinic were retrospectively screened, and it was determined that there were 1622 chronic hepatitis B patients, 679 chronic hepatitis C patients, and 78 Wilson's disease patients. In these patients, there were 218 chronic hepatitis B patients, 202 chronic hepatitis C patients, and 37 Wilson patients with computed tomography images. When evaluated according to inclusion criteria, 48 chronic hepatitis B patients, 56 chronic hepatitis C patients and 35 Wilson patients were included in the study. Results:The median age of the patients was 55 years. According to the mean value of the pancreas(tomographically average densities of the head, body and tail of the pancreas), pancreatic fat was found to be significantly higher in patients with a diagnosis of chronic hepatitis C (CHC) compared to patients with a diagnosis of chronic hepatitis B (CHB) and Wilson's disease (WD) (p:0.003). When the patients were evaluated according to pancreas mean, pancreatic spleen difference (Density value obtained by subtracting spleen density from pancreatic mean value) and pancreatic spleen ratio (pancreas mean density and spleen density ratio), no significant correlation was found between fatty liver and liver fat in three parameters, respectively (p:0.389)(p:0.277)(p:0.590). The erythrocyte sedimentation rate (ESR) was found to be significantly higher in the group with adiposity according to the mean pancreatic value (p:0.025). According to the mean value of the pancreas, urea and TSH were found to be significantly lower in the group with adiposity compared to those who did not (p:0.024)(p:0.011). According to the pancreas-spleen difference, globulin was found to be significantly higher in the group with adiposity compared to those who did not (p:0.043). According to the mean for pancreatic steatosis, spleen difference and spleen ratio, the mean age of the group without pancreatic steatosis was found to be lower in each internal evaluation (p:0.001)(p:0.023)(p:0.001). Conclusion:When the general results were examined, it was determined that there was significant pancreatic fat in CHC patients compared to the other two disease groups (according to the mean pancreas and pancreatic spleen ratio) (p=0.003 p=0.039). According to all three evaluation methods, with pancreatic fat; while a significant relationship was found between age (p<0.05), no statistically significant relationship was found with fatty liver (p>0.05). According to the pancreatic mean method, a significant relationship was found between pancreatic fat and BMI (p=0.029).
