The effect of initial periodontal treatment and diod laser decontamination treatment on gingival crevicular fluid TNF-A and IL-1B in patients with choronic periodontitis

Abstract (EN)

Chronic periodontitis is an infectious disease characterized by pathogen bacterial accumulation around the teeth and the resultant mast cell response is a simple gingival inflammation that begins with gum and periodontium destruction. Studies have been accepted as the gold standard for periodontal treatment without surgery. Mechanical therapy alone may not be predictable. These limitations affect long-term outcomes of non-surgical periodontal therapy and open the way for supportive therapies. The purpose of the study is; initial periodontal treatment in patients with chronic periodontitis, and the effect of decontamination treatment with this diode laser treatment on clinical parameters and the amount of Tnf-a and IL-1B in gingival fluid. A total of 30 subjects diagnosed with chronic periodontitis were included in the study. The study group consisted of patients with at least 1 (5>mm) periodontal pocket in each half-upper jaw posterior region. In the study designed as splint-mouth, mechanical treatment was applied to the right region, mechanical treatment + diode laser decontamination procedures were applied to the left region. Clinical parameters including plaque and gingival index, probable pocket depth and clinical attachment level were evaluated and ELISA test was used to determine the amount of gingival fluid Tnf-a and IL-1B. The results were compared between baseline, week 1, month 1, month 3. A statistically significant reduction in clinical parameters was observed when the baseline and third baseline data were compared in both treatment regions. The change in clinical parameters between groups at 3 months after treatment was found statistically insignificant. The change in Tnf-a and IL-1B levels in the laser treated area was significant when comparing baseline and third-month DOS Tnf-a and IL-1B values in both treatment zones, but only the change in the periodontally treated area was not significant. There was no statistically significant difference between the groups after the treatment, but the difference between the values was found to be significant. It has been observed that laser therapy in addition to non-surgical periodontal therapy has early clinical contributions to clinical parameters and pro-inflammatory cytokines, but does not contribute to long-term outcomes.
