Comparison of optic nerve sheath diameters in laparoscopic and laparotomic abdominal surgery

Aim: The aim of our study is to investigate the possible effects of laparoscopic and laparotomy abdominal surgeries on intracranial pressure changes monitored by ultrasonographic measurement of optic nerve sheath diameter, which is a highly reliable and practical method that does not require invasive intervention. Material and Method: 54 patients aged between 18 and 65 who underwent elective abdominal surgery in the operating room of Dicle University Hospital were included in the study. Exclusion criteria from the study were; Patients who did not want to participate in the study, ASA classification IV patients, emergency surgeries, cases with anesthesia duration over 180 minutes, pregnant women, patients with malignant hypertension (>180/110 mmHg), patients with any disease or history of medication that affects intraocular and intracranial pressure. Results: Optic nerve sheath diameter (ONSD) was measured 10 minutes after intubation, at the 30th minute of surgery and before extubation, and statistical comparisons were made between the measurements. In the measurements made at the 30th minute of surgery, the mean value of the measurements in group 1 was 5.50±0.43 mm, and in group 2, it was 5.09±0.31 mm. In these measurements made at the 30th minute of surgery, a statistically significant increase was found in group 1 compared to group 2. (p= <0.001) Conclusion: We believe that measuring optic nerve sheath diameter by ultrasonography can be used in diagnosis and intraoperative follow-up in patients with a possibility of elevated ICP, especially in laparoscopic abdominal surgeries. However, we believe that further scientific studies are needed to evaluate the effectiveness and reliability of this method in different patient groups and with larger sample groups, especially in individuals who have undergone laparoscopic abdominal surgery and have increased intracranial pressure.
