In this study,it is aimed to compare the patient controlled sedation with dexmedetomidine infusion and doctor-controlled sedation method in terms of hemodynamical effects,total medicine consumption,sedation level,post operative recovery periods and patient and doctor satisfaction during the surgery of breast biopsy under local anaestesie.40 patients aged between 18-60 and belonging to ASA I- II groups were randomly divided into two and were included in the study.None of the patients were premedicated.Patients who were taken into the operation room were monitorized EKG in the derivation of DII,pheripheral oxygen saturation(SpO2) and non-invasive blood pressure.Basal measurements were taken after monitorization in the operation room.Before local anaestesie,both of the groups were given loading dose with infusion pump in 10 minutes 1 mcg/kg dexmedetomidine.Basal infusion was applied about at an interval of0.2-0.7 mcg/kg/hour.For a conscious sedation, after giving medicine infusions in ten minutes,by anaesthetizing locally,the operation started when the target sedation level was reached.In the patient ?controlled group,the patient used 4 mcg/ml additional bolus prepared by himself before the operation when he needed.In the doctor controlled group,the anaesthetist applied additional bolus when there was a % 25 increase in tension arteriel,%20 increase in heart beats,agiation and ache.During the operation and at the end,HR, MBP, SpO2, NRS and sedation scores of the patients were measured and recorded.Successful and unsuccessful demand number, the total amount of the medicine used, the patients and the surgeons satisfaction and RSS 2 duration end the operation were recorded.In this study of ours in which we compared the patient controlled sedation with dexmedetomidine infusion and doctor-controlled sedation so as to establish a conscious sedation,we observed that the patients could recover in a shorter time by providing enough sedation and with a little drug consumption and a beter hemodynamic stability the peroperative period with the help of the patient controlled sedation without paving a way for a depper sedation.
