Men with and without metabolic syndrome in patients with lower urinary tract symptoms of alpha-blockers effectiveness of treatment

In the recent years, metabolic syndrome draws attention as an important etiological factor in the development of lower urinary tract syndrome (LUTS) in addition to the other risk factors. In this study, the effect of alfuzosin treatment in patients with lower urinary tract symptoms with and without metabolic syndrome is evaluated with comparison of plasebo control.Between May 2011 and May 2012, a total of 68 patients presented to Urology and Endocrinology outpatient clinics with LUTS were divided into four groups as having metabolic syndrome or not. These groups were: MetS patients treated with alfuzosin (Xalfu 10 mg, Generica-İstanbul)(Group 1), MetS patients receiving placebo (Group 2), non MetS patients treated with alfuzosin (Group 3), and non MetS patients taking placebo (Group 4). Evaluation was performed by taking detailed history, digital rectal examination and completing internationally validated questionnaires before and after the therapy. Metabolic syndrome was diagnosed according to the criteria of the National Cholesterol Education Program Adult Treatment Panel III. Uroflowmetry, postvoiding residual volume measurement, iNOS, TNF-?, COX-2 and bioavailable testosteron (bT) analysis were also performed.In our study, the mean Qmax values in the groups 1 and 3 before ?-blocker therapy were 12,19±2,13 and 10,59±2,63, respectively. After treatment, there was a %32,8 (16,25±5,48), and %30,1 (13,32±3,33) increase in groups 1 and 3, respectively. The decrease in IPSS in group 1 and 3 were 6,7 (-%37,7) and 5,1 (-%27,8), respectively, compared to placebo groups. The percentage of decrease in the IPSS-QoL (IPSS-YKS) were detected as -%21,5 and -%20,3 in the groups 1 and 3 respectively. Decrease in BPH-QoL (BPH-YKÖ) score was -%50,1 in group 1 and -%42,6 in the group 3. There was no statistically significant difference between the groups due to prostate and postvoiding residual volumes, bT, IEFF scores , iNOS, TNF-? and COX parameters before and after therapy.In the present study, it was determined that, ?-blocker drug treatment led to significant improvement of symptoms in patients with and without metabolic syndrome having LUTS when compared to placebo.
