LOOKING FOR THE LEVELS OF UROTENSIN-2 IN BLOOD AND SALIVA OF THE NEWLY DIAGNOSED AND METFORMIN USING DIABETES MELLITUS PATIENTS Diabetes Mellitus (DM); The primary symptom of carbohydrate metabolism with a chronic course is a multifactorial pathogenesis of the etiopathogenesis, characterized by a complete or partial deficiency of insulin or insulin resistance, a high plasma glucose level, and a heterogeneous clinical picture with significant changes in fat and protein metabolism. Although life expectancy is still low in diabetic patients, prognosis has improved in the last decade. The degree and duration of hyperglycemia play an important role in the development of complications. Metformin is the most commonly prescribed drug in antidiabetic drugs in the United States and is one of the most widely used drugs worldwide. Metformin reduces plasma glucose levels by reducing insulin resistance and glucose production in the liver. In patients with Type 2 DM, monotherapy can be used alone or in combination with other antidiabetic drugs or insulin. There is no risk of hypoglycemia because it is not an oral antidiabetic agent that makes insulin secretion. Urotensin-2 (UII), one of the most potent vasoconstrictor peptides known, is even stronger than endothelin-1, which is a prominent vasoconstrictor. It has peptide sequences that are similar to somatostatin. UII has been shown to cause vasodilation in healthy volunteers while hypertension and heart failure are affected by vasoconstriction. Plasma levels were found to be very low when measured in UII-radioimmunoassay method in healthy people. (5±1 fmol/ml). Therefore, some researchers believe that urotensin-2 is not a hormone circulating in plasma. With the activation of the UII system, there are studies suggesting that metabolic syndrome may develop. The aim of our study was to determine the levels of urotensin-2 in blood and saliva after treatment with type 2 metformin and to compare and compare with healthy volunteers. There are several studies in the literature showing that there is a relationship between diabetes mellitus and urotensin-2. However, there is no study in the literature regarding the effects of urotensin-2 after metformin treatment in Type 2 DM patients. It was thought that treatment with metformin might play a role in the pathogenesis of urotensin-2. In our study, 30 new diagnoses were compared with 30 volunteer healthy groups. There was no statistically significant increase in urotensin-2 levels in blood and saliva in new diagnosis Type 2 DM patients compared to the control group. New diagnosis Type 2 DM patients had a statistically significant increase in urotensin-2 levels in blood and saliva after 3 months of treatment with metformin, and urotensin-2 levels were found to be increased (p<0.05). In conclusion, after treatment with metformin, urotensin-2 levels are increased in patients with Type 2 DM who have chronic course of multifactorial pathology. Metformin has no known role in the pathogenesis of urotensin-2, one of the most potent vasoconstrictor peptides known. Further studies are needed to confirm our findings.
