Abstract (EN)

Non-invasive mechanical ventilation (NIMV) treatment is a treatment method in which respiratory support is maintained by a mask for the patients suffering from respiratory insufficiency.A significant majority of the studies about NIMV treatment is related with the effect of this treatment method on the prognose of the patient. However, studies focused on the problems which patients faced with in the treatment process and nursing care requirements are not at a sufficient amount in the literature. The study submitted was planned in order to determine the problems and nursing care requirements of the patients who receive NIMV treatment in intensive care units of general and cardiovascular surgery in a private hospital in the city of Istanbul, on the basis of relational screening model. Research was conducted with 20 patients who were admitted to the intensive care units between 26th of March and 26th of May (2014), were receiving NIMV treatment and accepted to participate in the research voluntarily. Data of the research were collected with 4 forms: Patient Data Collection Form, Nurse Observation Form, Patient Feedback Evaluation Form intended for NIMV Application and Additional Workload Determination Form associated with NIMV Treatment for Nurses which all were prepared by referring to the literature knowledge and clinical experience. Data were evaluated with t-test, chi-square and Spearman's correlation analysis at a 95% confidence interval and p<0.05 significance level by the help of software, SPSS (Statistical Package for Social Sciences) for Windows 17.0. The average age of patients was 65.6 and 70-percent was male, 50-percent of the cases were identified as lung-originated and 65-percent indicates respiratory insufficiency which arose in hypoxemic-type. Statistically significant improvements were detected for all blood values in the process of NIMV treatment (p<0.05). The average of mask adaptation points of the patients was 2.8 and the average for the comfort points was 3.4 at the 48th hour. Also there were observed some problems which can be classified according to their origins as skin in the 50-percent, eye in the 45-percent, nasal in the 60-percent and sleeping in the 40-percent among the patients. There were inferred no meaningful relationship between nurse observations and patient experiences in terms of problems associated with NIMV treatment (p<0.05). A 1.8-hour for additional-nurse-workload was deduced as a consequence of NIMV treatment. Keywords: Non-invasive complications, nursing care, additional workload
