Guidance services to students' self-realization, according to the capabilities and features of the educational process at the highest level and to exploit their potentials for optimal use and development is where the studies are conducted. Guides teachers working in school's Guidance service, who are graduated at faculty of university that areas of counseling and guidance. For exploring the latent powers or abilities and characteristics in order to benefit from the highest level of students are applied with surveys/scales by school guidance teachers. Schools applied in this survey/scale is usually on paper. The questionnaires administered on paper surveys/scale up the implementation of the evaluation are experiencing some problems in the process. These problems; scale by the students complete and correctly filled Failure to all students is not reached, the time and space limitation is the availability of student tracking difficulty of the evaluation errors in the assessment process takes a long time and paper waste is more like the problems encountered as is. The purpose of this study, the questionnaires with school guidance services survey/scale up the implementation of the assessment is directed to overcoming the problems encountered in the process. For this purpose, "School Guidance Services for Online Decision-Support System" software has been developed. With the aim to check the reliability of the software, we used this software at Yunus Emre Technical and Industrial Vocational High School for 3 years and this software has been verified by applying the software's reliability. In order to evaluate software in terms of functionality and design "Teacher Opinion Survey" and the "Student Opinion Survey" was created. Teacher Opinion Poll located in the five items related to teachers' demographic information, 34 items are related to teachers' opinions about the system. Student Opinion Survey consists of 37 items, five of these substances demographic information about students, 32 are related to students' opinions about the system. Developing scales applied at Yunus Emre Technical and Industrial Vocational High School, that studying 200 students and 48 classroom/school guidance an than teachers and students opinions about "School Guidance Services for Online Decision - Support System" were taken. The obtained data were subjected to statistical processing. When we look at this result on average in each of the two surveys "Agree" corresponds to the range. This results in terms of functionality and design of the developed software that shows a good level.
