Retrospective examination of ferritin, sedimentation, vitamin-D, annual painful crisis, liver and kidney function tests in patients with sickle cell anemia

Purpose: Sickle cell anemia is a heterogeneous hereditary disease characterized by anemia, which is autosomal recessive, develops as a result of damaged synthesis of Hb protein, the molecule that carries oxygen to the tissues. In this study, we examined the laboratory findings and its relationship with painful crisis, and the number of painful crises in patients with sickle cell anemia. Methods: The study was conducted on a total of 40 patients, 23 male and 17 female, diagnosed with sickle cell anemia. Demographic information, educational status, laboratory results and number of painful crises of the patients were obtained from the hospital registry system. Results: It was determined that 42.5% of the patients were female and 57.5% were male. It was determined that 35% of the patients were under the age of 20, and 65% of them were aged 20 and over. It was determined that 7.5% of the patients were primary school graduates, 25% secondary school graduates, 52.5% high school graduates and 12.5% university graduates. mean ferritin values of patients 706.88 ± 1202.66 ml/ng, mean sedimentation values 18.45 ± 21.39 mm/hour, mean CRP values 15.17 ± 28.96 mg/L and mean vitamin D values 13.80 It was determined as ± 6.02ng/ml. The mean ALT values of the patients were 27.92 ± 24.26 U/L, the mean of the AST values were 39.25 ± 20.30U/L, the mean of the ALP values were 124.63 ± 70.95U/L, the mean GGT values were 47.72 ± 64, It was determined that 83U/L, bilirubin values average 2.31 ± 1.72 mg/dL, creatinine values average 0.50 ± 0.17 mg/dL and Urea values 19.44 ± 8.05 mg/dL. The mean pain crisis years of the patients were found to be 3.26 ± 1.90 years. The mean annual pain crisis number of patients was determined as 3.29 ± 2.17 years in women and 3.23 ± 1.72 years in men. The mean annual pain crisis number of the patients was found to be 3.15 ± 2.41 years for those under 20 years of age, and 3.31 ± 1.64 years for those aged 20 and over. The mean annual pain crisis number of the patients was determined as 3.92 ± 2.56 v years in those with pre-high school education and 2.92 ± 1.44 years in those with high school and beyond. There was no statistically significant difference between the annual pain crisis numbers according to the demographic characteristics of the patients (p>0.05). According to the results obtained; A weak positive correlation was found between the patients' ferritin (r=0.354) and CRP (r=0.328) values and the number of annual pain crises (p<0.05). There was no statistically significant relationship between the hemoglobin F, hemoglobin S, vitamin D and sedimentation values of the patients and the annual number of pain crises.(p>0.05) Conclusion: In our study, we did not detect a gender and educational status difference between painful crisis and using care containment. It was determined as the point where ferritin and hs-CRP values increased at the edges with painful crisis. Although those who did not detect an association with painful attacks of vitamin D and HbF Values, similar ones were shown with certain vitamin D and HbF level painful crises.
