Investigation of the volumetric changes of the recipient fields in atrophic alveolar crests that are vertically and horizontally augmented with Autogeneous Ramus Block Graft, Poly (Lactic-co-Glycolicacide) (PLGA) block graft

Insufficient alveolar ridge height and width is one of the most common problems in dental and implant treatments planned for partially or completely edentulous patients. There are surgical approaches that make it possible to reconstruct these atrophic areas in the alveolar bone using various techniques and bone grafts. Autogenous block grafts taken from the ramus region are frequently preferred because they have advantages such as the quality of the obtained bone being sufficient, shortening the operation time due to being an intraoral donor site, and not having a high risk of complications. Poly Lactic-co-Glycolic Acid (PLGA)(bonegraft powerbone), one of the most popular materials in tissue engineering recently, is a synthetic material with rapid biodegradation and high biocompatibility, which has been approved by the FDA for clinical use in humans. In our study; The records of patients with uni/bilateral horizontal/vertical deficiency in the upper and/or lower jaw, who were operated on to reshape the alveolar bone, and who had cone beam computed tomography (CBCT) images before and at the 6th month after the operation were examined. Meets the inclusion criteria; A total of 26 patients, 5 men and 21 women, aged between 31 and 64, and 42 jaw areas were included in the study. Our work; It was conducted on two groups: PLGA, which is an alloplastic block graft (19 jaws of 11 patients) and autogenous ramus block graft (23 jaws of 15 patients). Belonging to the operation area; Preoperative and postoperative 6th month cone beam computed tomography (CBCT) images were examined; Volume and horizontal/vertical measurements were made. When the data obtained from our study are evaluated; The volume increase at the end of the 6th month was (28.3%) in the PLGA group and (29.2%) in the ramus block graft group. At the end of the 6th month of horizontal bone increase; It was found that the average length was 1.87±1.13 mm in the PLGA block graft group and 1.98±1.11 mm in the ramus block graft group. Vertical bone increase was observed in only 3 patients-6 jaws; The average length was 3.49±2.42 mm in the PLGA group and 2.55±1.26 mm in the ramus block graft group. When the results of our study are examined; At the end of the 6-month augmentation period in both groups; It was determined that sufficient amount and quality of bone thickness was obtained, but there was no significant difference between the two groups in terms of bone volume and horizontal bone increase.
