Pancreati̇c neuroendocri̇ne tumors:Factors affecting tumor recurrence and survi̇val

Objective: This study was undertaken to examine prognostic factors in patients with pancreatic neuroendocrine tumors and compare our data with the literature. Materials and Methods: In this study, between the years of 2007-2014, patients, treated at the Ege University Faculty of Medicine Hospital General Surgery Clinic were investigated retrospectively. Demographic data of the patients, surgical treatment shapes, postoperative recurrence and survival situations, pathological data were evaluated. Tumors were classified on the basis of 2010 WHO classification Guidelines and TNM system. Immunohistochemical staining was performed. Statistical analyzes were performed using IBM SPSS version 22.0 statistical software package. In calculating of survival rate the Cox regression and Kaplan-Meier methods were used. Results: A total of 33 patients with a mean age of 52,9 ± 2,11 years were included. Thirty of the tumors (90.9%) nonfunctional and 3 (9.1%) were functional. 52% of patients underwent distal pancreatectomy. 39% 's Whipple operation, 9%'s enucleation was performed. Average tumor size was calculated as 1.3 cm. Ki-67 average was found to be 9.81%. Recurrence, were found in 17 (51.5%) patients. Five-year overall survival was 81.8% (SE ± 0.035), respectively. No correlation was found between lymph node metastases and recurrence. The positive surgical margins and multifocality as other parameters were found to be unrelated to recurrence (p values> 0.05). Conclusion: Ki-67 and the number of mitosis is associated with both survival and recurrence. WHO classification in determining the prognosis is more effective than the TNM classification. Those with metastatic disease in the liver, it can be achieved close survival to nonmetastatic with proper surgical treatment. even if negative surgical margins is not achieved, patients can safely benefit from surgery.
