The effect arm ergometry exercises on disease violence and ambulation in parkinson pati̇ents

Abstract (EN)

Parkinson's disease (PD) is a progressive neurodegenerative disease characterized by degeneration of dopaminergic cells in substantia nigra, the second most common disease after Alzheimer's disease. Traditional treatment methods are used in PD rehabilitation and many physical exercise types such as treadmill and bicycle training have been investigated in the rehabilitation of Parkinson's patients. In this study, we aimed to investigate the role of arm ergometer exercises in the rehabilitation of Parkinson's Disease, the severity of the disease and the contribution of ambulation, and the duration of contribution. Thirty-three patients completed a prospective randomized controlled trial. Patients included in the study were divided into two groups by stratified randomization. There was no statistically significant difference between the two groups in terms of demographic data, disease duration, comorbidities, disease severity, levodopa use. The first group had traditional rehabilitation and arm ergometer exercises. The second group was only included in the traditional rehabilitation program. Patients were evaluated 3 times in total before treatment, after treatment, 3.months 2 hours after taking your meds with 6-minute walk test, Modified Borg Scale during 6-minute walk test, five times sit-to-stand test, right and left hand grip strength, Berg Balance Score, United Parkinson's Disease Rating Scale. Rehabilitation programs were organized for 5 weeks and 25 sessions for 5 weeks. Training protocol was planned as 16 minutes in the first week for the arm ergometer group. The training programs were reorganized for each week. The patients were evaluated before treatment, after treatment and at 3 months. Significant improvements in 6 minutes walking tests, Modified Borg Scales, 5 times sit-up test, right and left hand grip strength, Berg balance scores, and all parameters of UPDRS in short periods in the group receiving arm ergometry compared to the pre-treatment group respectively. In the conventional group, there was a statistically significant increase except for in right-left hand grip strength and part 4 of the UPDRS. When the group receiving conventional treatment with arm ergometer before treatment, after treatment and after 3 months of treatment was compared with the group receiving only conventional treatment, the arm and conventional group were significantly higher in 2, 3 and total UPDRS than in the conventional treatment group at both post-treatment and 3rd month. In the 3-month evaluations, the Modified Borg Scale, the Berg Balance Scale and the UPDRS part 1 showed significant improvement in the arm ergometer group compared to the conventional group. In the arm ergometry group, 6-minute walking test, Modified Borg Scales during the 6-minute walk test, five sit-to-stand tests, right and left hand grip strength, and the effectiveness of the UPDRS part 2 and part 4 were still in the third month. We believe that arm ergometer exercises should be considered as a part of the Parkinson's Disease rehabilitation program and should be effective in the rehabilitation of this disease as it is used in many diseases.
