Renal functional changes after percutaneous nephrolithotomy and the analyses of the factors effecting these changes

Özet (EN)

It is aimed that in this study analyses of the renal functional changes after Percutaneous Nephrolihotomy (PNL) and the factors which effects this changes.In this study PNL was performed in 80 patients (48 male 32 female) with kidney stone. Mean age of the patients was 43.71 (18-71) years. Laterality on patients were 38 (47.5%) in the right kidney, 42 (52.5%) in the left kidney. Preoperative and postoperative stone sizes, Glomerular Filtration Rate (GFR), Dimercaptosuccinic acid (DMSA), serum creatinine, urea, electrolytes (Na, K, Cl), and Hb values were compared, perioperative blood transfusion were recorded. Cockroft-Gault method was used in the calculation of GFR.In postoperative period, a statistically significant decrease was observed in stone size, serum urea and creatinine levels. A statistically significant increase was observed on GFR values.There were no changes observed on the serum electrolytes (Na, K, Cl) values. 13 patients were required blood transfusion (%16.3). Transfusion rate was not affected with stone size, patient age and Body Mass Index (BMI), but long operation time appeared to increase the rate of transfusion. Another factor increasing the rate of transfusion was the multipl access. Postoperative stone free rate, multipl access, operation time and the type of lithotripter didn?t effect the GFR changes.Today in the light of findings, PNL is highly reliable and efficient method in stone surgery, because it can supply improvement on kidney functions by increasing GFR and high stone-free rate.
