Özet (EN)

Placental invasion anomalies have rapidly been growing due to the increase in caesarean deliveries in the last 50 years. Placenta percrata which is the most advanced form of placenta invasion anomalies is a life-threatening condition associated with significant maternal morbidity and mortality. American College of Obstetricians and Gynecologists (ACOG); not only recommends planned caesareanhysterectomy by leaving the placenta in place due to the risk of haemorrhagic morbidity but also specifies the personalization of each case for surgical management. Since fertility preservation is certainly a concern subject, various conservative methods have been tried in the literature. The loss of the uterus of the patient means loss of the most important aspect of feminine character in some of the culture. New conservative interventions can be effective in improving women's psychological and social conditions that affect their quality of life. In our study; by temporarily clamping the common iliac artery which feeds the uterus, together segmental of the anterior wall of the uterus in patients with placenta percrata, we aim to reduce the amount of bleeding and the need for blood products during operation and perform uterine preserving surgery in patients who want to protect the uterus or want to have child. Our study is prospectively designed in patients with placenta percrata who applied to our clinic between May 2022 and May 2023. While the common iliac arteries were temporarily clamped in 19 of the patients, uterine segmental resection was performed in 17 patients by applying uterine preserving surgery. In our study not include the one which servical invasion and parametrial invasion patients As a result of our study, we observed that there is significant difference in the amount of bleeding between patients who underwent segmental resection by clamping the common iliac arteries in patients with placenta percrata and those who underwent segmental resection.
