Evaluation and importance of the diagnostic value of platelet variability index in cerebrovascular events

Abstract (EN)
Background-Aim: Stroke is a clinical picture of vascular origin, which develops suddenly without any reason, presents with focal or generalized cerebral dysfunction, may be temporary or permanent and sometimes with mortality. Since mortality and morbidity rates due to acute stroke are high, it is important to predict early mortality and prognosis in these patients. In our study, by looking at MPV and PLT values and MPV / PLT ratio, which is an easily accessible laboratory test, whether there is a relationship between the patients' service, intensive care admission or discharge status, there is a relationship between blood pressure values and MPV / PLT ratios. It was evaluated in what way this ratio was affected in the age groups over 65 and under 65. In addition, it was evaluated whether there was a relationship between the triage code of the patients at the time of admission and MPV, PLT, MPV / PLT values. Material-Method: The patients who applied to Manisa Celal Bayar University Medical Faculty Hafsa Sultan Hospital Emergency Service between 01.11.2019-01.09.2020 and were diagnosed with non-traumatic cerebrovascular hemorrhage and ischemic stroke as a result of the examinations were retrospectively evaluated. Results: Considering the demographic characteristics of the participants; The study group consisted of 116 male (58%) 84 female patients. The average age of the participants was found to be 68.14. 200 people participating in the study were evaluated, 178 patients were diagnosed with ischemic stroke (89%) and 22 patients with hemorrhagic stroke (11%). A weak correlation was found between the patients' age and MPV / PLT ratios (p <0.05). Systolic and diastolic blood pressure of the patients are evaluated; A weak correlation was found between MPV / PLT ratio and systolic blood pressure values, it is statistically significant (p <0.05). The relationship between MPV-PLT and MPV / PLT values was evaluated, no statistically significant difference was found. No statistically significant relationship was found between the gender of the patients and the patients' PLT-MPV and MPV / PLT values (p> 0.05). A statistically significant relationship was not found between the MPV-PLT and MPV / PLT values between the patient groups over 65 years of age and patients under 65 years of age (p> 0.05). MPV, PLT and MPV / PLT ratios were evaluated in the ischemic and hemorrhagic stroke groups, no statistically significant relationship was found between the two groups (p> 0.05). Triage codes are evaluated in a similar way; No relationship was found between triage codes and MPV / PLT values. Conclusion: In our study, we tried to evaluate the relationship between MPV, PLT and MPV / PLT values of patients admitted to the emergency department with ischemic stroke or hemorrhagic stroke, and the demographic characteristics, triage code, outcome, vital parameters and GCS scores of the patients. In our study, a correlation was found between patients 'systolic blood pressure and MPV / PLT ratios. In addition, a weak correlation was found between the patients' age and MPV / PLT ratios. No statistically significant correlation was found between the triage code and GCS scores of the patients at the time of admission and MPV / PLT ratios. No statistically significant correlation was found between the MPV / PLT ratios of patients with the patients.
