Evaluation of metabolic parameters and adiposity indices according to phenotypes in polycystic ovary syndrome

Abstract (EN)

Objective: We aimed to compare the metabolic parameters and adiposity indices of polycystic ovary syndrome patients by dividing them into four different phenotypes. Material and Methods: Participants in this study (n=200) were evaluated according to the presence of hyperandrogenism (H), oligo/anovulation (O) and polycystic ovary appearance (P); were grouped into four phenotypes as phenotype A (H+O+P), phenotype B (H+O), phenotype C (H+P) and phenotype D (O+P). Participiants were examined for waist-hip measurements, body mass indices, ultrasonic examination, modified Ferriman-Gallwey scoring, endocrine hormone profiles, lipid profiles, androgen levels. Results: Metabolic syndrome was found in 15% (n=30) of the patients. While these four phenotypes were evaluated among themselves, no significant difference was observed in metabolic syndrome. While the mean body mass index was calculated as 26.43 ± 5.21 kg/m2 in all patients, the highest mean value was found in phenotype A. The waist/hip ratios of the patients were found to be similar in all groups. While mean systolic blood pressure values were similar in all four groups, mean diastolic blood pressure values were found the highest in phenotype D and the lowest in phenotype B. Total cholesterol mean values were found to be the highest in phenotype D and the lowest in phenotype B. HOMA-IR and QUICKI values were found to be similar in all four groups. The lowest HDL and highest LDL mean values were found in phenotype A. LAP (lipid accumulation product) and VAI (visceral adiposity index) values were found to be significantly higher in phenotype A compared to other groups. Conclusion: Phenotype A and phenotype D have a higher risk in metabolic and cardiovascular diseases. It was determined that phenotype B had the lowest cardiometabolic risk.
