The evaluation of the effect of hypericum perforatum oil on fibrinolitic activity in postoperative surgical adhesions in rats

Abstract (EN)
Postoperative adhesions are a clinical problem with serious complications such as intestinal obstruction, infertility, and abdominal and pelvic pain. A definitive treatment method has not been found yet, despite the research. A healty fibrinolytic system is required to prevent postoperativa adhesions. The fibrinolytic system is activated by the rise of tissueplasminogen activator (tPA) and ends with the inhibition of plasminogen activator inhibitor 1 and 2 (PAI-1 and 2). In our study, we aimed to evaulate the effect of Hypericum Perforatum oil that is stated to be effective in burns and wound healing on fibrinolytic activity and postoperative adhesion formation. The rats were divided into three groups as the control group, olive oil group and Hypericum Perforatum group. Each group consisted of seven rats. Group 1 was comprised of the control group with the adhesion model only. In group 2, an adhesion model was formed and olive oil was applied. Group 3 involved an adhesion model and the application of Hypericum Perforatum oil. Rats cared for and handled under appropriate conditions were sacrificed after 14 days and evaluated according to adhesion strength and area classification scores.peritoneal fluid and cardiac blood and tissue samples were taken from the rats for the ELISA measure ment and histopathological evaluation. There was no statistical difference between the rat groups in the analysis of the adhesion area and strength (P>0,05). TPA and PAI-1 levels were measured by the ELISA method and their effectiveness on the fibrinolytic system was evaluated. The comprasion of the tPA values between the groups revealed no significant difference in terms of serum, peritoneal fluid and tissue levels in all experimental groups compared to the control group (P>0,05). When PAI-1values were compared between groups, there was a significant difference in the rat group that received Hypericum Perforatum oil in the peritoneal fluid compared to the control group (P<0,05). Histopathological examinaations revealed no significant results in inflammation and fibrinolysis evaluations (P>0,05). The results of our study suggested that the application of Hypericum Perforatum oil could not be effective in preventing fibrinolysis due to high PAI-1 in the peritoneal fluid and thus adhesion formation.
