The preventive role of resveratrol and quercetin in severe acute pancreatitis

ABSTRACT THE PREVENTIVE ROLE OF RESVERATROL AND QUERCETIN IN SEVERE ACUTE PANCREATITIS Acute pancreatitis, occurs due to abrupt pancreatic inflammation. In proportion to the severity of disease despite therapy severe complications and mortality rates are high. In 15-20% of patients with acute pancreatitis can be terminated with life-threatening illness, long hospitalization, surgical intervention or death. In patients with severe pancreatitis, shock, acute renal failure, acute respiratory failure, multiorgan failure, and even death can be seen. Because of ıt ıs an inflammatuar disease, studies focused on natural ingredients with antiinflammatory property. Resveratrol and quercetin has anti-inflammatory and antioxidant effects, in this study we investigated their protective effects on severe acute pancreatitis. İn this study in seven groups total of 42 female Wistar rats were used. Group1 (n=7) (K) basal diet applied. Intraperitoneal DMSO (dimetil sülfoksit) applied to group 2 (n=7) (DMSO). In the group 3 (n=7) (C) 20 mcg/kg cerulein administered subcutaneously (sc) 4 times at intervals of 1 hour. In the group 4 (n=7) (C+R) 20 mcg/kg cerulein administered subcutaneously (sc) 4 times at intervals of 1 hour, 30 mg/kg resveratrol+DMSO administered intraperitoeal before first cerulein dose. Grup 5 (n=7) (C+Q) 20 mcg/kg cerulein administered subcutaneously (sc) 4 times at intervals of 1 hour, 50 mg/kg quercetin administered by orogastric gavege before first cerulein dose. In the group 6 (n=7) (C+R+Q) 20 mcg/kg cerulein administered subcutaneously (sc) 4 times at intervals of 1 hour, 30 mg/kg resveratrol+DMSO intraperitoneal, 50 mg/kg quercetin by orogastric gavage administered before fist cerulein dose. After completing the study protocol rats were decapitated under anesthesia. Pancreatic tissue samples were taken for biochemical and histopathological examination. Amylase, lipase, AST and ALT were studied from serum samples. Pancreatic tissue was stained with hematoxylin and eosin after fixation. It assessed in therms of edema, acinar necrosis, fat necrosis, leukocyte infiltration and classified according to histopatological scoring critteria. Pancreatic tissue was homogenized. MDA studied by HPLC method, TNF alfa, COX-2, NFκB and IκB level studied by real time PCR method. End of the study compared to control group, serum amylase levels were significantly higher in groups C, C+RES and there was no significant difference between control grup and C+QUE, C+RES+QUE groups. In terms of the level of serum lipase significant difference revealed between control and cerulein group. There was no significant difference between control and group 4, 5, and 6. Serum amylase, lipase levels and histopatological scores concerned group 5 (C+QUE) and group 6 (C+QUE+RES) have improved serum amylase, lipase levels and histopatological scores significantly. Histopathological scores improved by both quercetin and resveratrol. The combined use of resveratrol and quercetin wasn't more effective than the only use of quercetin. Resveratrol, quercetin and the combined administration reduced the level of MDA, TNF alfa, COX-2, NFκB and the combined administration prevented the decrease in the level of IκB. Both resveratrol, quercetin and their combined use reduced the severity of disease. Keywords: Severe acute pancreatitis, cerulein, resveratrol, quercetin
